三阴性乳腺癌JMJD3和PTEN的表达与临床病理特征的关系

摘　　要：目的三阴性乳腺癌侵袭性强、预后差,对其分子标记物的研究有助于诊断和预后判断,三阴性乳腺癌中JMJD3和PTEN的相关性尚不清楚,本研究旨在观察三阴性乳腺癌组织中JMJD3和PTEN蛋白水平的表达,分析二者的相关性及其与临床病理特征的关系,拓展对三阴性乳腺癌发生的理解与认识,为乳腺癌治疗和预防提供新策略。方法选取80例非三阴性乳腺癌组织,87例三阴性乳腺癌和相对应的癌旁组织,用免疫组织化法检测JMJD3和PTEN蛋白的表达情况,采用半定量方法进行判读,用χ2检验分析JMJD3和PTEN与三阴性乳腺癌年龄、肿瘤部位、分化程度、淋巴结转移及TNM分期的关系。用Spearman法分析JMJD3和PTEN的相关性。采用Kaplan-Meier法来评估JMJD3和PTEN蛋白的异常表达与三阴性乳腺癌生存期之间的关系。结果三阴性乳腺癌中JMJD3和PTEN蛋白的表达率34.48%(30/87)、40.23%(35/87)分别显著低于癌旁组织74.71%(65/87)、68.97%(60/87)和非三阴性乳腺癌组织75.00%(60/80)、72.50%(58/80)(P<0.05)。在87例三阴性乳腺癌中,JMJD3和PTEN分别与组织学分化、淋巴结转移状况及TNM分期有关(均P<0.05)。JMJD3在高分化中的表达率76.00%(19/25)高于中-低分化17.74%(11/62),在有淋巴结转移中13.33%(6/45)低于无淋巴结转移57.14%(24/42),其在TNMⅠ+Ⅱ期组别66.67%(20/30)高于Ⅲ+Ⅳ期组别17.54%(10/57),差异均有统计学意义(均P<0.05)。PTEN在高分化中的表达率76.00%(19/25)高于中-低分化25.81%(16/62),其在淋巴结有转移组别中22.22%(10/45)低于无淋巴结转移组别59.52%(25/42),其在TNMⅠ+Ⅱ期组别70.00%(21/30)高于Ⅲ+Ⅳ期组别24.56%(14/57),差异均具有统计学意义(均P<0.05)。Spearman相关分析结果显示JMJD3和PTEN呈显著正相关(Rs=0.539,P<0.001)。Kaplan-Meier分析结果显示JMJD3蛋白高表达者的无病生存率86.7%(26/30)高于低表达者43.4%(23/53),PTEN蛋白高表达者无病生存率82.9%(29/35)高于低表达者41.7%(20/Objective Three negative breast cancer has strong invasiveness and poor prognosis.The study of its molecular markers is helpful for the diagnosis and prognosis judgment.The correlation between JMJD3 and PTEN in three negative breast cancer is not clear.The purpose of this study was to observe the expression of JMJD3 and PTEN protein in three negative breast cancer,analyze the correlation between them and clinicopathological characteristics and prognosis.It also expanded the understanding of the occurrence of triple negative breast cancer,which provided a new strategy for the treatment and prevention of breast cancer.Methods The expression of JMJD3 and PTEN in non three negative breast cancer(80 cases),three negative breast cancer(87 cases)and their corresponding adjacent tissues were detected by immunohistochemistry.The expression of JMJD3 and PTEN were detected by semi quantitative method.The relationship between JMJD3/PTEN and age,tumor location,differentiation,lymph node metastasis and TNM stage of three negative breast cancer was analyzed by Xz test.The correlation between JMJD3 and PTEN was analyzed by Spearman method.Kaplan Meier method was used to evaluate the relationship between the abnormal expression of JMJD3/PTEN protein and the survival of triple negative breast cancer.Results The positive rate of JMJD3 and PTEN protein in three negative breast cancer was 34.48%(30/87)and40.23%(35/87),which were significantly lower than 74.71%(65/87),68.97%(60/87)in paracancerous tissue and 75.00%(60/80)and 72.50%(58/80)in non three negative breast cancer tissue(P<0.05).In 87 cases of triple negative breast cancer,JMJD3 and PTEN were related to histological differentiation,lymph node metastasis and TNM stage(P<0.05).The expression rate of JMJD3 in high differentiation was 76.00%(19/25)higher than17.74%(11/62)in medium to low differentiation,lymph node with metastasis was 13.33%(6/45)lower than57.14%(24/42)in lymph node without metastasis,TNMⅠ+Ⅱgroup was 66.67%(20/30)higher than 17.54%(10/57)inⅢ+Ⅳgroup,and the
