摘  要:目的探讨轻度胃肠炎伴婴幼儿良性惊厥(BICE)患儿血清一氧化氮水平、一氧化氮合酶(NOS)活性、可溶性白细胞介素2受体(SIL-2R)、白细胞介素(IL)-6、IL-8的水平及临床意义。方法纳入89例轻度胃肠炎患儿(单纯肠炎组)、36例BICE患儿(BICE组)、30例健康婴幼儿(正常对照组)。检测对照组体检时,以及其他两组入院24 h内(急性期)、发病后5~7 d(恢复期)血清一氧化氮水平、NOS活力以及SIL-2R、IL-6、IL-8水平。结果(1)在急性期,BICE组、单纯肠炎组血清一氧化氮水平、NOS活力均高于正常对照组(均P<0.05),且BICE组以上指标均高于单纯肠炎组(均P<0.05),但在恢复期两组差异无统计学意义(均P>0.05);BICE组恢复期血清一氧化氮水平、NOS活力均较急性期降低(均P<0.05),而单纯肠炎组急性期与恢复期以上指标差异无统计学意义(均P>0.05)。(2)在急性期,BICE组、单纯肠炎组血清SIL-2R、IL-6、IL-8水平均高于正常对照组(均P<0.05),但在急性期及恢复期两组间以上指标差异无统计学意义(均P>0.05);BICE组、单纯肠炎组恢复期血清SIL-2R、IL-6、IL-8水平均较急性期降低(均P<0.05)。结论BICE患儿血清一氧化氮水平及NOS活力增高,可能是引起惊厥的原因之一;BICE患儿机体存在免疫失衡,动态监测SIL-2R、IL-6、IL-8水平有助于评估其免疫功能紊乱纠正情况及病情变化。Objective To explore serum nitric oxide level,nitric oxide synthase(NOS)activity,soluble interleukin 2 receptor(SIL-2R),interleukin(IL)-6 and IL-8 levels,and their clinical significance in mild gastroenteritis children accompanied by benign infantile convulsions(BICE).Methods Eighty-nine children with mild gastroenteritis(simple enteritis group),36 children with BICE(BICE group)and 30 healthy children(normal control group)were selected.Serum nitric oxide level,NOS activity,and levels of SIL-2R,IL-6 and IL-8 were determined in the control group during physical examination,and in the remaining two groups within 24 hours of admission(acute phase)and 5 to 7 days after onset(recovery phase).Results(1)In acute phase,serum nitric oxide level and NOS activity in the BICE group and the simple enteritis group were higher than those in the normal control group(all P<0.05),and the indices mentioned above were higher in the BICE group than in the simple enteritis group(all P<0.05),but showed no statistically significant difference between the two groups in recovery phase(all P>0.05);in the BICE group,serum nitric oxide level and NOS activity were declined in recovery phase than in acute phase(all P<0.05),but the indices mentioned above exhibited no statistically significant differences between the acute phase and the recovery phase in the simple enteritis group(all P>0.05).(2)The BICE group and the simple enteritis group possessed higher levels of serum SIL-2R,IL-6 and IL-8 in acute phase as compared with the normal control group(all P<0.05),but no statistically significant differences were found between the two groups in the indices mentioned above in acute phase or recovery phase(all P>0.05);in the BICE group and the simple enteritis group,serum levels of SIL-2R,IL-6 and IL-8 were decreased in recovery phase than in acute phase(all P<0.05).Conclusion Serum nitric oxide level and NOS activity are increased in children with BICE,which may be one of the causes of convulsion;immune dysregulation exists in children with BICE,and
