下颌下腺瓣在口腔颌面部软组织缺损修复中的应用效果

摘　　要：目的探讨下颌下腺瓣(SMGF)在口腔颌面部软组织缺损修复中的应用效果。方法对2010年1月—2020年1月于我院口腔颌面头颈肿瘤科采用SMGF修复肿瘤切除后软组织缺损的34例患者的临床资料进行回顾性分析。结果34例患者中,男20例,女14例;平均年龄(65.5±0.3)岁;良性肿瘤13例,恶性肿瘤21例。32例患者术后2周出院时SMGF均愈合良好;2例患者术后1周出现咽瘘,经换药以后延期愈合。术后随访平均58个月(3个月~10年),1例恶性肿瘤患者手术后2年因肿瘤复发转移死亡;2例患者手术后1年局部复发再次接受手术治疗,扩大切除包括SMGF在内的组织后未再随访;其余31例患者随访期间均无明显功能障碍。结论采用SMGF修复口腔内小范围软组织缺损是一种可靠的方法。Objective To investigate the application of submandibular gland flap(SMGF)in the repair of soft tissue defect in the oral and maxillofacial region.Methods A retrospective analysis was performed for the clinical data of 34 patients with soft tissue defect after tumor resection who underwent the repair with SMGF in Department of Oral-Maxillofacial Head and Neck Oncology in our hospital from January 2010 to January 2020.Results Among the 34 patients,there were 20 male patients and 14 female patients,with a mean age of(65.5±0.3)years,and among these patients,13 had benign tumor and 21 had malignant tumor.All 32 patients achieved good healing of SMGF at the time of discharge at 2 weeks after surgery.Two patients experienced pharyngeal fistula at 1 week after surgery and had delayed healing after dressing change.The mean postoperative follow-up was 58 months(3 months to 10 years).One patient with malignant tumor died of tumor recurrence and metastasis at 2 years after surge-ry;2 patients underwent surgical treatment for local recurrence at 1 year after surgery and no follow-up was performed after exten-ded resection of the tissue including SMGF;the remaining 31 patients had no obvious dysfunction during follow-up.Conclusion SMGF is a reliable method for repairing small soft tissue defect in the oral cavity.
