广东省江门市HIV-1亚型及流行毒株gag基因特征分析

摘  要:目的了解广东省江门市1型艾滋病病毒(HIV-1)基因亚型分布状况及其主要流行株CRF01AE的传播和基因变异特征。方法收集江门市2018-2019年新确证为HIV-1感染且未经抗病毒治疗的208例样本,采用巢式荧光定量聚合酶链反应法(PCR)扩增gag基因序列并测序,应用Genotyping Tool进行基因分型、PhyML3.0软件构建系统进化树,利用Entropy和VESPA程序对CRF01AE毒株亚簇进行氨基酸序列特征分析。结果成功测得gag序列173条,异性性传播占72.83%(126条),同性性传播占17.92%(31条);亚型分析结果显示,CRF01AE 69例(39.88%),CRF07BC 57例(32.95%),19例(10.98%)CRF5501B,12例(6.94%)B亚型,其他亚型包括CRF08BC、CRF5901B、C亚型和URFs共16例(9.25%)。CRF01AE在系统进化树上形成4个传播簇,其中2个主要传播簇共有11个氨基酸位点的特征性氨基酸分布存在显著差异,且传播簇I的氨基酸多态性大于传播簇Ⅱ。结论江门市HIV-1亚型分布复杂多样,以异性性传播为主,存在多种传播簇。CRF01AE亚型为江门地区的优势流行株,其亚簇的氨基酸存在变异。Objective To explore the distribution of HIV-1 subtypes and identify HIV-1 epidemic as well as genetic variation of CRF01AE in Jiangmen city,Guangdong province.Methods Blood samples were collected from 208 newly defined and treatment na?ve HIV-1 infected individuals,with local residence in Jiangmen from 2018 to 2019.HIV-1 gag gene was amplified by nested polymerase chain reaction(PCR)and then the subsequent products were carried out for sequencing.HIV genotype was identified by viral genotyping tool and the PhyML3.0 software was used to construct the phylogenetic tree.The variation of aminoacid of CRF01AE sub-clusters were analyzed by entropy and VESPA program in HIV database.Results A total of 173 gag gene sequences were successfully obtained.Of all subjects,72.83%(126/173)were heterosexually transmitted and 17.92%(31/173)were homo-sexually transmitted.The distribution of HIV-1 genotypes was 69(39.88%)CRF01AE,57(32.95%)CRF07BC,19(10.98%)CRF5501 B,12(6.94%)subtype B,and other subtypes included CRF08BC,CRF5901 B,subtype C and URFs in 16 cases of 9.25%(16/173).Four sub-clusters were found in the CRF01AE phylogenetic tree.Further analysis of the two main evolution clusters indicated that there were 11 specific aminoacid alterations among them,with the lower aminoacid polymorphism of cluster II compared with cluster I.Conclusion There are complex and diverse HIV-1 subtypes in Jiangmen with kinds of molecular clusters and heterosexual contact as the main transmission.CRF01AE is the predominant epidemic strain and there is variation in aminoacids of the sub-clusters.
