二维斑点追踪纵向应变评价蒽环类药物对乳癌病人左心房功能影响

摘　　要：目的应用二维斑点追踪纵向应变评价蒽环类药物对乳癌病人左心房功能的影响并探讨其临床意义。方法选取以蒽环类药物化疗为主的乳癌病人52例,均行6周期化疗,分别于化疗前及化疗第2、4、6周期行二维斑点追踪技术检查,检测左心房整体纵向应变(GLS)、左心房储备期峰值应变(SRs)、左心房通道期峰值应变(SRe)及左心房辅泵期峰值应变(SRa)。以同期30例健康志愿者为正常对照组。结果 乳癌病人化疗第4、6周期的左心房GLS、SRs、SRe及SRa均低于对照组及乳癌病人化疗前、化疗第2周期,化疗第6周期的左心房GLS、SRs、SRe、SRa较第4周期降低,差异有统计学意义(F=21.044~53.702,P<0.05)。结论 应用二维斑点追踪技术评估蒽环类药物对射血分数正常的乳癌病人的左心房功能影响有一定临床价值。ObjectiveTo investigate the value of two-dimensional speckle tracking and longitudinal strain in evaluating the influence of anthracyclines on left atrial function in breast cancer patients.MethodsA total of 52 breast cancer patients who received chemotherapy based on anthracyclines were enrolled and were all treated for 6 chemotherapy cycles.Before chemotherapy and at cycles 2,4,and 6 of chemotherapy,two-dimensional speckle tracking was performed to measure the global longitudinal strain(GLS),systolic strain rate(SRs),early diastolic strain rate(SRe),and atrial systolic strain rate(SRa)of the left atrium.A total of 30 healthy volunteers were enrolled as normal control group.ResultsIn the breast cancer patients,the GLS,SRs,SRe and SRa at cycles 4 and 6 of chemotherapy were significantly lower than those before chemotherapy and at cycle 2 of chemotherapy,and the breast cancer patients had significantly lower GLS,SRs,SRe and SRa than the normal control group at cycles 4 and 6 of chemotherapy;there were reductions in the GLS,SRs,SRe and SRa of the left atrium from cycle 4 to cycle 6 of chemotherapy(F=21.044-53.702,P<0.05).ConclusionThe two-dimensional speckle tracking technique has a certain clinical value in evaluating the influence of anthracyclines on left atrial function in breast cancer patients with normal ejection fraction.
