摘 要:目的探讨幽门螺杆菌(Hp)感染与肠上皮化生(IM)的相关性。方法对132368例胃镜检查病人临床资料进行回顾性分析,统计IM检出率;应用多因素Logistic回归分析确定IM的影响因素;比较IM病人与无IM胃炎病人的Hp感染率,以及轻、中、重度IM病人的Hp感染率。结果 132368例病人中发生IM者39805例,检出率为30.1%。多因素Logistic回归分析显示,Hp感染(OR=1.487,95%CI=1.449~1.527,P<0.01)、年龄(OR=1.038,95%CI=1.037~1.039,P<0.01)、男性(OR=1.264,95%CI=1.234~1.286,P<0.01)均为IM的危险因素。IM病人的Hp感染率为35.2%,明显高于无IM的胃炎病人(29.5%),差异有统计学意义(OR=1.294,95%CI=1.262~1.328,P<0.01);轻、中、重度IM病人的Hp感染率分别为37.1%、33.6%、21.9%,各组之间比较差异均有统计学意义(χ2=240.759,P<0.01)。结论 Hp感染、性别、年龄均是IM发生的危险因素,随着IM程度的加重Hp感染率呈下降趋势,可能与中、重度IM病人的胃内微环境不适合Hp生存有关。ObjectiveTo investigate the association between Helicobacter pylori(Hp)infection and intestinal metaplasia(IM).MethodsA retrospective study was performed for the clinical data of 132368 patients who underwent gastroscopy,and the detection rate of IM was calculated.A multivariate Logistic regression analysis was used to determine the influencing factors for IM.Hp infection rate was compared between gastritis patients with or without IM,and Hp infection rate was compared between the patients with mild,moderate or severe IM.ResultsAmong the 132368 patients,39805 had IM,with a detection rate of 30.1%.The multivariate Logistic regression analysis showed that Hp infection(OR=1.487,95%CI=1.449-1.527,P<0.01),age(OR=1.038,95%CI=1.037-1.039,P<0.01),and male sex(OR=1.264,95%CI=1.234-1.286,P<0.01)were risk factors for IM.The gastritis patients with IM had a significantly higher Hp infection rate than those without IM(35.2%vs 29.5%,OR=1.294,95%CI=1.262-1.328,P<0.01),and the Hp infection rates were 37.1%,33.6%,and 21.9%,respectively,in the patients with mild,moderate or severe IM(χ2=240.759,P<0.01).ConclusionHp infection,sex,and age are risk factors for IM.Hp infection rate tends to decrease with the aggravation of IM,which may be associated with the unsuitable microenvironment for Hp survival in patients with moderate or severe IM.
