子宫内膜癌组织中HIF-1α、Bcl-2、β-catenin的表达与临床生物学行为和预后的关系探讨

摘　　要：目的探讨子宫内膜癌组织中缺氧诱导因子-1α(HIF-1α)、Bcl-2、β-连环素(β-catenin)的表达与临床生物学行为和预后的关系。方法选择78例子宫内膜癌患者为研究对象,同期选择因单纯宫颈上皮内瘤变切除子宫的正常增殖期内膜组织69例,全子宫切除或刮宫的不典型增生子宫内膜组织73例。观察HIF-1α、Bcl-2、β-catenin表达情况。结果子宫内膜癌HIF-1α、Bcl-2、β-catenin阳性表达率依次高于不典型增生子宫内膜及正常子宫内膜(P<0.05)。组织病理分级低分化、子宫肌层浸润≥1/2、FIGO分期Ⅲ~Ⅳ期、有淋巴结转移组HIF-1α、Bcl-2、β-catenin阳性表达率均分别高于组织病理分级高分化、子宫肌层浸润<1/2、FIGO分期Ⅰ~Ⅱ期、无淋巴结转移组(P<0.05)。HIF-1α、Bcl-2、β-catenin阳性表达率者5年生存例数均分别低于HIF-1α、Bcl-2、β-catenin阴性表达率者(P<0.05)。结论子宫内膜癌组织中HIF-1α、Bcl-2、β-catenin的表达明显上调,能够反映肿瘤的临床生物学行为,可作为子宫内膜癌预后评价的辅助指标。Objective To investigate the relationship between expression of hypoxia inducible factor-1α(HIF-1α),Bcl-2,β-catenin(β-catenin)with clinical biological behavior and prognosis in endometrial carcinoma.Methods 78 patients with endometrial cancer were selected as the research group,69 patients with normal proliferative endometrium were selected for hysterectomy due to simple cervical intraepithelial neoplasia,73 patients with atypical proliferative endometrium were selected for hysterectomy or curettage.The expressions of hif-1,bcl-2 and-catenin were observed.Results The positive expression rates of HIF-1α,Bcl-2 andβ-catenin in endometrial carcinoma were higher than those in atypical hyperplasia endometrium and normal endometrium(P<0.05).The positive expression rates of HIF-1α,Bcl-2,β-catenin in the cases of histopathological grade low differentiation,myometrial invasion≥1/2,FIGO stageⅢ~Ⅳ,lymph node metastasis were higher than those in histopathological grade high differentiation,myometrial invasion<1/2,FIGO stageⅠ~Ⅱ,and no lymph node metastasis group(P<0.05).The number of 5-year survival cases with positive expression rates of HIF-1α,Bcl-2 andβ-catenin were lower than those with negative expression rates respectively(P<0.05).Conclusion The expression of HIF-1α,Bcl-2,β-catenin in endometrial carcinoma tissue is significantly up-regulated,which can reflect the clinical biological behavior of the tumor and can be used as an auxiliary index for prognosis evaluation of endometrial carcinoma.
