摘 要:目的通过扫描电镜观察刷牙实验前后氟钾酚醛树脂脱敏剂(fluorine potassium phenolic resin desensitizer,FR)、激光、激光联合氟钾酚醛树脂脱敏剂对牙本质小管的封闭情况,探讨不同脱敏方法对牙本质小管的封闭效果及抗洗刷能力。方法选择新鲜拔除的第三磨牙48颗,要求牙冠完整、无龋损,制备成3.0 mm×3.0 mm×2.5 mm的牙本质模块,根据表面脱敏方法不同随机分为4组:A组对照组;B组为氟钾酚醛树脂脱敏剂组;C组激光组;D组为激光联合氟钾酚醛树脂脱敏剂组。B、C、D 3组脱敏处理后,每组均随机选择6个标本作为即刻组,其余6个标本作为磨损组,进行刷牙实验。每组随机选择一半标本沿牙本质方向纵行劈开,用于观察纵剖面。扫描电镜观察4组样本牙本质小管的表面封闭情况及纵剖面牙本质小管的封闭深度,所得的数据均在SPSS 24.0软件上进行统计学分析。结果①B、C、D 3组均能在牙本质表面形成有效封闭(P<0.05),D组具有更佳的封闭效果。②纵剖面观察,B、D两组封闭深度差异无统计学意义(P>0.05),均深于C组(P<0.05)。③刷牙磨耗试验后B、C、D 3组牙本质小管口的封闭效果有统计学差异(P<0.05),两两比较,D组表面封闭性优于B、C两组(P<0.05),B组优于C组(P<0.05);④纵剖面封闭深度比较,B、D两组均大于C组(P<0.05)。结论①氟钾酚醛树脂脱敏剂、激光、激光和氟钾酚醛树脂脱敏剂联合应用均能在牙本质表面形成有效封闭。②激光和氟钾酚醛树脂脱敏剂联合应用对牙本质小管的封闭效果要优于单用脱敏剂或激光,更能耐受机械性刷洗,效果持久而稳定;激光封闭牙本质小管的深度较小,且抗机械性刷洗的能力较差。Objective To observe the sealing effect of potassium fluoride resin desensitizer,laser and laser combined with potassium fluoride phenolic resin desensitizer on dentinal tubles by scanning electron microscope,to explore the sealing effect of dentin tubule and anti-brushing ability of different desensitization methods,so as to provide experimental basis and theoretical support for the clinical application of laser combined with fluoropotassium phenolic resin desensitizer.Methods Forty-eight caries-free third molars were selected and prepared into 3.0 mm×3.0 mm×2.5 mm dentin blocks.They were randomly divided into four groups according to different desensitization methods.Group A:control group;Group B:potassium fluoride phenolic resin desensitizer group;Group C:laser group;Group D:laser combined with fluoropolyphenolic resin desensitizer group.In group B/C/D,6 samples were randomly selected as immediate group and the remaining 6 samples were selected as wear group for the brushing experiment.Half of the dentin specimens were randomly selected to be split in each group to observe the longitudinal profile.The surface closure of the dentinal tubules of the blank group and the test group was observed by scanning electron microscope.The morphological analysis was performed in the representative areas to observe the closure depth of dentin tubules in longitudinal profile.SPSS 24.0 was used to analyze all the data.Results①Group B,group C and group D could effectively block the dentin surface(P<0.05),while group D had better sealing effect.②In pairwise comparison,there was no statistical difference in the depth of closure between group B and D(P>0.05).However,both were deeper than group C(P<0.05).③After brushing tooth,the differences among the three groups in sealing dentin tubule orifice were significant(P<0.05).Group D was better than group B and group C,while group B was better than group C(P<0.05).④After brushing tooth,for the depth of longitudinal section,group B and group D were deeper than group C(P<0.05).C
