摘　　要：目的探讨术前纤维蛋白原(fibrinogen,FIB)与女性三阴性乳腺癌总生存期(overall survival,OS)的关系。方法2009年12月~2012年12月我院收治的女性三阴性乳腺癌病人295例。采用回顾性研究,并采用Kaplan-Meier法及Cox比例回归风险模型进行生存分析,Log-rank检验分析组间差距。探讨术前FIB表达与女性三阴性乳腺癌病人总生存期(overall survival,OS)之间的关系。结果使用X-Tile软件找到FIB的最佳切断值,将病人分成两组,FIB<3.7 g/L组和FIB≥3.7 g/L组。使用Kaplan-Meier法单因素分析,FIB≥3.7 g/L组中,OS明显降低(HR,2.4,95%CI,1.1,4.9,P=0.002);使用COX风险回归模型多因素分析,在FIB≥3.7 g/L组中,OS也显著降低(HR,2.2,95%CI,1.1,4.6,P=0.028)。利用多因素分析建立的Kaplan-Meier生存曲线结果显示术前FIB值越高,OS越低(Log-rank检验,P=0.028)。结论术前FIB低表达是女性三阴性乳腺癌OS的独立预后因素。Objective To explore the relationship between preoperative fibrinogen(FIB)and overall survival Overall Survival(OS)of women with triple negative breast cancer.Methods A total of 295 women with triple-negative breast cancer treated in the Breast Center of the Fourth Hospital of Hebei Medical University from December 2009 to December 2012 were collected.A retrospective study was adopted,and Kaplan-Meier method and Cox proportional regression risk model were used for survival analysis.Log-rank test was used to analyze the differences between groups.To investigate the relationship between preoperative FIB expression and OS in female triple negative breast cancer patients.Results X-Tile software was used to find the best cut-off value of FIB(g/L),and the patients were divided into two groups,FIB<3.7 g/L group and FIB≥3.7 g/L group.Using the Kaplan-Meier method for univariate analysis,OS was significantly reduced in the FIB≥3.7 group(HR,2.4,95%CI,1.1,4.9,P=0.002);using the COX risk regression model for multivariate analysis,in the FIB≥3.7 group In addition,OS was also significantly reduced(HR,2.2,95%CI,1.1,4.6,P=0.028).Kaplan-Meier survival curve results established by multivariate analysis showed that the higher the preoperative FIB value,the lower the OS.Conclusion Low preoperative FIB expression is an independent prognostic factor for overall survival in women with triple negative breast cancer.
