从火郁发之理论探析升降散治疗失眠症

摘　　要：自《黄帝内经》提出火郁发之理论以来,历代医家对此续有阐发。"火郁"二字重在"郁"字,有"郁"固当"发","发之"核心思想在于因势利导,给邪以出路。纵观"火郁发之"之病机,无非气血失衡,从心、肝入手,舒畅气机。火郁的治疗,宜因势利导,宣畅气机,使其外达。升降散其辨证运用的关键是气机失调,无论寒热虚实,只要是气机逆乱,都可采用升降散加减变化来调整脏腑功能气机,达到气血阴阳的协调。失眠症多因气机郁滞所致,因此从火郁发之论治升降散治疗失眠具有广泛的临床指导意义。Since the theory of expelling fire constraint was put forward in the Huangdi Neijing,Chinese medicine practitioners of all dynasties have continued to elucidate this theory."Fire constraint"emphasizes on"constraint"and it is bound to"expel"the"constraint".The key to"expelling"is to make the best use of the circumstances to give way to pathogenic factors to go out.In terms of the pathogenesis of method of"expelling fire constraint",it is to treat qi-blood imbalance from the heart and liver to soothe qi movement.That is to say,the treatment of fire constraint should make the best use of the circumstances and diffuse and soothe qi movement to let fire constraint externally expelled.The key to syndrome differentiation of Shengjiang power is disorder of qi movement;no matter patients are with cold,heat,deficiency or excess syndrome,as long as their qi movement is counterflow,they can be treated with modified Shengjiang power to regulate the qi movement of all zang-fu organs,so as to achieve the harmony of qi,blood,yin and yang.Insomnia is caused by stagnation of qi.Therefore,it is of great clinical significance to treat insomnia from the theory of expelling fire constraint.
