百合固金汤加减联合顺铂胸腔内注射治疗非小细胞肺癌合并恶性胸腔积液临床研究

摘　　要：目的:观察百合固金汤加减联合顺铂胸腔内注射治疗非小细胞肺癌(NSCLC)合并恶性胸腔积液(MPE)的疗效。方法:选择102例NSCLC合并MPE患者,随机分为对照组和观察组各51例。对照组采用顺铂胸腔内注射治疗,观察组采用百合固金汤加减联合顺铂胸腔内注射治疗,28 d为1个疗程,连续治疗2个疗程。比较2组治疗前后中医证候评分、生活质量评分、免疫功能及疗效。结果:治疗后,观察组CD4^+、CD4^+/CD8^+、免疫球蛋白A(IgA)、免疫球蛋白G(IgG)、免疫球蛋白M(IgM)均较治疗前升高,且观察组CD4^+、CD4^+/CD8^+、IgA、IgG、IgM高于对照组(P<0.05);观察组CD8+降低,且观察组CD8+低于对照组(P<0.05)。治疗后,2组中医证候评分均降低,且观察组中医证候评分低于对照组(P<0.05);2组生活质量评分均升高,且观察组生活质量评分高于对照组(P<0.05)。对照组总有效率为43.14%(22/51),观察组总有效率为64.71%(33/51),观察组总有效率明显高于对照组,差异有统计学意义(P<0.05)。结论:百合固金汤加减联合顺铂胸腔内注射治疗NSCLC合并MPE疗效显著,能够缓解肺部症状,明显改善患者免疫功能和生活质量。Objective:To observe the curative effect of modified Baihe Gujin tang combined with intrapleural injection of cisplatin for patients with non-small cell lung cancer(NSCLC)complicated with malignant pleural effusion(MPE).Methods:A total of 102 cases of patients with NSCLC complicated with MPE were selected and randomly divided into the control group and the observation group,51 cases in each group.The control group was treated with intrapleural injection of cisplatin,and the observation group was additionally treated with modified Baihe Gujin tang based on the treatment of the control group.Both groups were treated for two courses of treatment,28 days as a course.Before and after treatment,the scores of Chinese medicine syndromes and quality of life,the immune function and curative effect in the two groups were compared.Results:After treatment,the levels of CD4^+,CD4^+/CD8^+,immunoglobulin A(IgA),immunoglobulin G(IgG)and immunoglobulin M(IgM)in the observation group were increased when compared with those before treatment and higher than those in the control group(P<0.05);CD8^+level in the observation group was decreased when compared with that before treatment and lower than that in the control group(P<0.05).After treatment,Chinese medicine syndrome scores in the two groups were decreased,and the score in the observation group was lower than that in the control group(P<0.05);the scores of quality of life in the two groups were increased,and the score in the observation group was higher than that in the control group(P<0.05).The total effective rate was 64.71%(33/51)in the observation group,significantly higher than that of 43.14%(22/51)in the control group,the difference being significant(P<0.05).Conclusion:The therapy of modified Baihe Gujin tang combined with intrapleural injection of cisplatin for NSCLC complicated with MPE has significant curative effect,can relieve lung symptoms and obviously improve the immune function and quality of life of patients.
