摘　　要：目的探讨外周血异常糖链糖蛋白(tumour abmormal protein,TAP)检测在肿瘤早期辅助诊断及肿瘤筛查中的意义。方法选取2019年1~9月山西白求恩医院肿瘤内科确诊为恶性肿瘤患者379例(肿瘤组),门诊、住院部被诊断为癌前病变的患者293例(癌前病变组),这些病例均曾自愿进行过TAP检测,记录检测结果,以同期体检中心曾行TAP检测的健康体检者1763例(对照组)为对照,分析比较TAP检测结果。结果对照组、癌前病变组、恶性肿瘤组TAP阳性率分别为1%,7%和82.3%,三组间TAP阳性率比较,差异有统计学意义(χ^2=1703.147,P<0.001);TAP对肿瘤辅助诊断的灵敏度、特异度、阳性预测值分别为89.7%,96.8%和87.3%,癌谱广;三组中TAP检测阳性率均与性别无关(χ^2=0.333~2.534,均P>0.05)。结论TAP检测灵敏度和特异度高,且不受性别限制,癌谱广,在健康体检肿瘤筛查中值得广泛应用,对肿瘤的早期辅助诊断具有参考价值。Objective To investigate the significance of detecting normal blood chain glycoprotein(tumour abmormal protein,TAP)in the early diagnosis and tumor screening of tumors.Methods From January to September 2019,379 patients(tumor group)were diagnosed as malignant tumors in the Department of Oncology of Shanxi Bethune Hospital,and 293 patients(precancerous lesion group)were diagnosed as precancerous lesions in the outpatient and inpatient departments.All of them had voluntarily conducted TAP test and recorded the test results.Based on the comparison of 1763 healthy people who had undergone TAP test in the physical examination center(control group)during the same period,the results of TAP test were analyzed and compared.Results The positive rates of TAP in the control group,precancerous lesion group and malignant tumor group were 1%,7%and 82.3%respectively.There was a statistically significant difference in the TAP positive rate among the three groups(χ^2=1703.147,P<0.001).The sensitivity,specificity and positive predictive value of tumor diagnosis were 89.7%,96.8%and 87.3%,respectively,and the positive rate of TAP test in the three groups was not related to gender(χ^2=0.333~2.534,all P>0.05).Conclusion The TAP test has high sensitivity and specificity for tumor diagnosis,is not limited by gender,and has a broad cancer spectrum.It is worthy of extensive application in tumor screening for healthy physical examination and has reference value for early auxiliary diagnosis of tumors.
