磁共振成像联合动态增强扫描序列对肿块型胰腺炎的诊断价值

摘　　要：目的探讨磁共振成像(MRI)联合动态增强扫描(DCE)序列对肿块型胰腺炎的诊断价值。方法选择2018年1月至2020年1月于我院就诊的120例胰腺肿块病变患者为研究对象,对其实施计算机断层扫描(CT)、MRI平扫、DCE-MRI序列检查,以穿刺活检病理诊断结果为参照,比较CT、MRI平扫、DCE-MRI序列检查对肿块型胰腺炎的诊断效能,并对不同诊断方法结果的一致性进行分析。结果根据穿刺活检病理诊断结果,DCE-MRI序列对肿块型胰腺炎的诊断灵敏度、特异度、准确度、阳性预测值、阴性预测值均高于CT、MRI平扫(P<0.05),而MRI平扫与CT的灵敏度、特异度、准确度、阳性预测值、阴性预测值无显著差异(P>0.05)。CT、MRI平扫检查结果与穿刺活检病理诊断结果之间具有中度一致性,DCE-MRI序列检查结果与穿刺活检病理诊断结果之间具有高度一致性。结论MRI联合DCE序列对肿块型胰腺炎的诊断价值良好,可准确鉴别区分肿块型胰腺炎与胰腺癌。Objective To investigate the diagnostic value of magnetic resonance imaging(MRI)combined with dynamic contrast enhanced scanning(DCE)sequence in mass pancreatitis.Methods A total of 120 patients with pancreatic mass lesions admitted in our hospital from January 2018 to January 2020 were selected as study objects,and computed tomography(CT),MRI scan and DCE-MRI sequence were performed on them.Based on the pathological diagnosis results of biopsy,the diagnostic efficiency of CT,MRI scanand DCE-MRI sequence examination in mass pancreatitis were compared,and the consistency of results of different diagnostic methods were analyzed.Results According to the pathological diagnosis results of biopsy,the sensitivity,specificity,accuracy,positive predictive value and negative predictive value of DCE-MRI sequence were higher than those of CT and MRI scan(P<0.05);there were no significant differences in sensitivity,specificity,accuracy,positive predictive value and negative predictive value between MRI scan and CT(P>0.05).There was moderate consistency between CT,MRI scan results and pathological diagnosis results of biopsy,and high consistenty between DCE-MRI and pathological diagnosis results of biopsy.Conclusion MRI combined with DCE sequence has a good diagnostic value for mass pancreatitis,which can accurately indentify and differentiate mass pancreatitis from pancreatic cancer.
