外阴癌46例临床特点及预后因素分析

摘　　要：目的探讨临床病理因素和治疗方法对外阴癌预后的影响。方法收集2009年1月—2013年12月福建省妇幼保健院收治的46例原发性外阴癌患者的临床资料,回顾性分析其预后。结果 46例外阴癌患者的5年生存率为78.3%。根据2009年FIGO的手术病理分期,Ⅰ期18例,Ⅱ期2例,Ⅲ期10例,Ⅳ期2例。Ⅰ~Ⅱ期和Ⅲ~Ⅳ期的5年生存率分别为80%和50%。两组之间的生存率差异有统计学意义(P <0.05)。32例患者接受了广泛的外阴切除术和腹股沟淋巴结切除/活检。淋巴结转移12例,无淋巴结转移20例。淋巴结转移和非淋巴结转移者的5年生存率分别为50%和100%。两组的生存率差异具有统计学意义(P <0.05)。结论腹股沟淋巴结转移和手术病理分期是影响外阴癌预后的不利因素。Objective To investigate the clinicopathological factors and treatment of vulvar cancer and its influence on prognosis.Methods The clinical data of 46 patients with primary vulvar cancer admitted to Fujian Maternal and Child Health Hospital from January 2009 to December 2013 were collected,and the prognostic factors were retrospectively analyzed.Results The 5-year survival rate of 46 patients with negative cancer was 78.3%.According to the 2009 FIGO surgical pathological staging,18 cases were stageⅠ,2 cases were stageⅡ,10 cases were stageⅢ,2 cases were stageⅣ,and the 5-year survival rates of stagesⅠtoⅡandⅢtoⅣwere 80%and 50%,respectively.The difference in survival rates between groups was statistically significant(P<0.05).Thirty-two patients underwent extensive vulvar excision and bilateral superficial inguinal lymph node resection or biopsy.Postoperative pathology was 20 cases without lymph node metastasis and 12 cases with lymph node metastasis.The 5-year survival rates were 100%and 50%,respectively.The difference in survival rate was statistically significant(P<0.05).Conclusion Surgical pathological staging and inguinal lymph node metastasis are adverse factors affecting the prognosis of vulvar cancer.
