摘  要:目的分析骨良性纤维组织细胞瘤的临床特点及影像征象,加深对该病的认识,以期提高影像诊断水平。方法选择2015年6月—2019年6月以来共12例经病理证实的骨良性纤维组织细胞瘤的患者,回顾性分析其临床资料及影像表现,并参考国内外相关文献进行分析总结。结果 12例均为单发病灶,其中10例发生在长管状骨,X线及CT检查表现为偏心或中心性溶骨性骨质破坏,边缘可见硬化边,部分可见骨皮质中断,出现病理性骨折时可见骨膜反应,周围无软组织肿块。10例行MRI检查,病灶表现为T1WI呈等低信号,T2WI呈等高信号或混杂信号,病灶周围见低信号带,病灶增强后可见明显强化。结论了解骨良性纤维组织细胞瘤的影像表现,结合临床特点,有助于该病的诊断及鉴别诊断。Objective To analyze the clinical features and imaging features of benign fibrous histiocytoma of bone,to deepen the understanding of this disease,to improve the level of imaging diagnosis.Methods The clinical data and imaging manifestations of 12 patients with bone benign fibrous histiocytoma confirmed by pathology from June 2015 to June 2019 were retrospectively analyzed,and combined with the recent years at home and abroad related literature analysis and synthesis.Results All the 12 cases were single lesions,10 of them occurred in long tubular bones.X-ray and CT examination showed eccentric or central osteolytic bone destruction,with sclerotic edges visible at the edges,and partial cortical disruption of bone.Periosteum reaction can be seen when pathological fractures occur,and there is no soft tissue mass around.10 routine MRI examinations showed that the lesions presented isohyper signal on T1 WI,isohyper signal or mixed signal on T2 WI,low signal bands were seen around the lesions,and obvious enhancement was observed after lesion enhancement.Conclusion Understanding the imaging features and clinical features of benign fibrohistiocytoma of bone is helpful for the diagnosis and differential diagnosis of this disease.
