消化性胃溃疡老年患者联用三联疗法与瑞巴派特治疗的效果

摘　　要：目的:探讨消化性胃溃疡老年患者联用三联疗法与瑞巴派特治疗的效果。方法:选取2018年1月—2019年3月我院消化内科收治的136例消化性胃溃疡老年患者为本次的研究对象,随机将其分为每组各有68例患者的研究组和对照组。对照组患者采用克拉霉素、阿莫西林和兰索拉唑的三联疗法治疗,在三联疗法治疗的基础上,研究组患者加服瑞巴派特。对两组的临床疗效进行比较,观察治疗前后两组的胃黏膜形态学及炎性细胞改善情况评分,检测并比较两组治疗前后炎症因子水平。结果:研究组与对照组的总有效率比较明显提高,差异有显著性(P<0.05)。治疗后两组活动性与慢性炎症细胞浸润、黏膜厚度以及腺体密度评分均明显低于治疗前,治疗后研究组上述指标评分较对照组降低,差异有显著性(P<0.05)。治疗后研究组IL-1、IL-6水平明显低于治疗前,治疗后研究组上述指标评分较对照组降低,差异有显著性(P<0.05)。结论:消化性胃溃疡老年患者联合应用三联疗法与瑞巴派特治疗可以取得满意的效果,患者胃黏膜形态学指标明显改善,机体炎症反应水平显著降低,推广应用的价值较高。Objective:To investigate the effect of triple therapy and rebamipide in elderly patients with peptic ulcer.Methods:136 elderly patients with peptic ulcer from January 2018 to March 2019 in the Department of Gastroenterology of our hospital were selected as the research objects,and they were randomly divided into study group and control group with 68 patients in each group.The control group was treated with clarithromycin,amoxicillin and lansoprazole triple therapy.On the basis of triple therapy,the study group was given rebamipide.The clinical efficacy of the two groups were compared.The gastric mucosal morphology and inflammatory cell improvement scores were observed before and after treatment,and the levels of inflammatory factors were detected and compared between the two groups before and after treatment.Result:The total effective rate of the study group was significantly higher than that of the control group(92.65%vs 80.88%)(P<0.05).After treatment,the scores of activity and chronic inflammatory cell infiltration,mucosal thickness and gland density in the two groups were significantly lower than those before treatment.After treatment,the scores of the above indexes in the study group were lower than those in the control group(P<0.05).After treatment,the levels of IL-1 and IL-6 in the study group were significantly lower than those before treatment,and the scores of the above indexes in the study group were lower than those in the control group after treatment,with significant difference(P<0.05).Conclusion:The combined application of triple therapy and rebamipide in elderly patients with peptic ulcer can achieve satisfactory results.The morphological indexes of gastric mucosa in patients with peptic ulcer are significantly improved,and the level of inflammatory reaction is significantly reduced.Therefore,the application of triple therapy in elderly patients with peptic ulcer is of high value.
