摘  要:目的:观察HPV L1壳蛋白在宫颈脱落细胞中的表达,探讨使用其临床应用价值。方法:选择2017年2月至2019年4月我院收治的高危型HPV阳性、阴道镜检查怀疑有宫颈病变的女性,从中选择210例患者的宫颈脱落细胞标本作为本次研究的对象。采用免疫细胞化学法检测HPV L1壳蛋白表达。观察其中160例高危型HPV阳性、阴道镜检查怀疑有宫颈病变女性的宫颈脱落细胞HPV L1壳蛋白表达检查结果。结果:本次检测中,HPV L1壳蛋白的总阳性率为40.00%。慢性宫颈炎、CINⅠ、Ⅱ、ⅢHPV L1壳蛋白阳性率存在明显的差异,具有统计学意义(P<0.05)。不同宫颈细胞学分组中HPV L1壳蛋白表达阳性率存在明显差异,HPV L1壳蛋白在NILM分组中阳性表达率最低。结论:HPV L1壳蛋白表达与宫颈脱落细胞病变类型存在明显的联系,HPV L1壳蛋白阳性表达与宫颈脱落细胞病变程度呈现负相关,采用免疫细胞化学法检测这一指标可作为临床预判宫颈病变进展风险的重要依据。Objective:To observe the expression of HPV L1 capsid protein in cervix exfoliated cells and explore its clinical application value.Methods:From February 2017 to April 2019,high-risk HPV-positive women admitted to our hospital and suspected of having cervical lesions by colposcopy were selected.210 patients with cervical exfoliated cell specimens were selected as the object of this study.The expression of HPV L1 capsid protein was detected by immunocytochemistry.Observe the results of 160 cases of HPV L1 capsid protein expression in cervical shed cells of women with high-risk HPV positive and colposcopy suspected of having cervical lesions.Results:In this test,the total positive rate of HPV L1 capsid protein was 40.00%.There were significant differences in the positive rates of chronic cervicitis,CINⅠ,Ⅱ,andⅢHPV L1 capsid protein,with statistical significance(P<0.05).The positive rate of HPV L1 shell protein expression in different cervical cytology groups was significantly different.The positive expression rate of HPV L1 shell protein was the lowest in NILM group.Conclusion:The expression of HPV L1 shell protein is significantly related to the type of cervical shed cell disease.The positive expression of HPV L1 shell protein is negatively correlated with the degree of cervical shed cell disease.The detection of this indicator by immunocytochemistry can be used as a clinical predictor of cervical disease progression An important basis for risk.
