吉西他滨联合奥沙利铂治疗复发耐药老年淋巴瘤患者的疗效观察

摘　　要：目的评价复发耐药老年淋巴瘤患者临床治疗中采取联合用药(吉西他滨+奥沙利铂)的预后效果,为复发耐药老年淋巴瘤治疗工作提供参考。方法选择50例复发耐药老年淋巴瘤患者,予以吉西他滨联合奥沙利铂治疗,观察患者治疗后疗效及用药安全性。结果50例患者治疗总有效率为76%(38/50)。患者治疗后的不良反应主要表现为胃肠道反应及骨髓抑制反应。胃肠道反应患者中,恶心呕吐总发生率为34%(17/50),腹泻总发生率为28%(14/50),口腔黏膜炎总发生率为18%(9/50),肝功能异常总发生率为18%(9/50),肾功能异常总发生率为8%(4/50)。骨髓抑制患者中,白细胞减少总发生率为34%(17/50),血小板减少总发生率为26%(13/50)。结论复发耐药老年淋巴瘤患者采取吉西他滨与奥沙利铂联合治疗效果、安全性尚可,临床工作中仍需不断研究和优化。Objective To evaluate the prognostic effect of gemcitabine and oxaliplatin in the clinical treatment of elderly patients with relapsed and drug-resistant lymphoma,so as to provide reference for the treatment of elderly patients with relapsed and drug-resistant lymphoma.Methods A total of 50 elderly patients with relapsed drug-resistant lymphoma were treated with gemcitabine combined with oxaliplatin.The efficacy and safety of the patients after treatment were observed.Results The total effective rate of treatment for 50 elderly patients with relapsed drug-resistant lymphoma was 76%(38/50).The adverse reactions of patients after treatment were mainly gastrointestinal reactions and bone marrow suppression reactions.Among patients with gastrointestinal reactions,the total incidence of nausea and vomiting was 34%(17/50),diarrhea was 28%(14/50),oral mucositis was 18%(9/50),abnormal liver function was 18%(9/50),and renal dysfunction was 8%(4/50);among patients with bone marrow suppression reactions,the total incidence of leukopenia and thrombocytopenia was 34%(17/50)and 26%(13/50)respectively.Conclusion The efficacy and safety of gemcitabine combined with oxaliplatin in the treatment of elderly patients with relapsed and drug-resistant lymphoma are acceptable,and continuous research and optimization are still needed in clinical work.
