摘  要:目的探讨维生素D对克罗恩病(CD)患者铁调节蛋白和炎性因子的影响。方法选择2017年10月至2019年10月南通大学附属建湖医院收治的102例CD患者,按照随机数表法随机分为治疗组(51例)和对照组(51例),另择102名同期体检的健康成年人作为健康组。对照组给予常规治疗,治疗组在给予与对照组相同治疗的基础上,给予维生素D治疗,疗程均为1个月。比较治疗组和对照组的治疗有效率、铁调节蛋白(转铁蛋白、铁蛋白、铁调素及血清铁)水平、肠黏膜屏障、氧化应激程度及肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、IL-1β及C反应蛋白(CRP)水平。比较3组的血清25羟基维生素D325(OH)D3水平。结果治疗组的总有效率高于对照组,差异有统计学意义(P<0.05)。治疗后,治疗组的转铁蛋白、铁蛋白、铁调素、血清铁、丙二醛(MAD)、D-乳酸、二胺氧化酶(DAO)、TNF-α、IL-6、IL-1β及CRP水平低于对照组,而超氧化物歧化酶(SOD)水平高于对照组,差异均有统计学意义(P均<0.05)。治疗后,健康组的血清25(OH)D3水平高于治疗组,治疗组的血清25(OH)D3水平高于对照组,差异均有统计学意义(P均<0.05)。治疗有效者的转铁蛋白、铁蛋白、铁调素、血清铁、MAD、DAO、D-乳酸、TNF-α、IL-6、IL-1β及CRP水平低于治疗无效者,而治疗有效者的SOD及血清25(OH)D3水平高于治疗无效者,差异均有统计学意义(P均<0.05)。结论CD患者存在明显的维生素D缺乏,及时补充维生素D可降低铁调节蛋白及炎性因子水平,促进患者康复。Objective This paper intends to explore the effect of vitamin D on iron regulatory protein and inflammatory factors in patients with Crohn′s disease(CD).Methods A total of 102 CD patients admitted to Jianhu Hospital Affiliated to Nantong University from October 2017 to October 2019 were randomly assigned to the treatment group(51 cases)and the control group(51 cases)based on the random number table method.Another 102 adults who had physical examinations during the same period were selected as the healthy group.The control group was given conventional treatment,while the treatment group was given vitamin D apart from the conventional treatment,for 1 month.The treatment efficiency,iron regulatory protein(transferrin,ferritin,hepcidin and serum iron)levels,intestinal mucosal barrier,oxidative stress,tumor necrosis factor-α(TNF-α),interleukin-6(IL-6),IL-1β,and C-reactive protein(CRP)levels of the treatment group and the control group were compared,so were the serum 25-hydroxyvitamin D3[25(OH)D3]levels of the 3 groups.Results The total effective rate of the treatment group was higher than that of the control group,with a statistically significant difference(P<0.05).After treatment,the transferrin,ferritin,hepcidin,serum iron,serum malondialdehyde(MAD),D-lactic acid,diamine oxidase(DAO),TNF-α,IL-6,IL-1β,and CRP levels were lower than those of the control group,and the level of superoxide dismutase(SOD)was higher than that of the control group,with a statistically significant difference(P<0.05).After treatment,the serum 25(OH)D3 level of the healthy group was higher than that of the treatment group,and that of the treatment group was higher than that of the control group,with a statistically significant difference(P<0.05).The transferrin,ferritin,serum iron,hepcidin,MAD,DAO,D-lactate,TNF-α,IL-6,IL-1βand CRP of the treatment-effective patients were lower than those of the treatment-ineffective patients,and the levels of SOD and serum 25(OH)D3 were higher than those of the treatment-ineffective patients,with a st
