摘 要:目的:研究无托槽隐形矫治器关闭邻面去釉间隙时牙齿位移情况及牙周组织的应力分布。方法:建立牙冠类型为方圆型的上颌前部牙列,牙槽骨,牙周膜及无托槽隐形矫治器的三维有限元模型,模拟临床关闭邻面去釉间隙,获得关闭间隙时牙齿位移及牙周组织的应力情况。结果:在关闭邻面去釉间隙时,上颌6个前牙位移表现为近中倾斜移动唇向倾斜伴随龈向压低,牙周组织无明显应力集中,牙颈部应力高于根中及根尖1/3,尖牙远中面应力相对较高。结论:无托槽隐形矫治器关闭邻面去釉间隙时,不会因应力集中而对牙周组织造成损失,牙齿表现为近中、唇向及龈向压低的移动。Objective:To study the tooth displacement and the stress distribution of periodontal tissue when the clear aligner closes the gap following interproximal enamel reduction.Methods:CBCT and MIMICS 19.0 ANSYS 15.0 software were used for the establishment of the overall 3D model of the square crown type maxilla,maxillary anterior teeth,periodontal ligament and clear aligner.The tooth displacement and the stress distribution of periodontal tissue were analyzed before and after space closure.Results:When the space of interproximal enamel reduction was closed,the displacement of the 6 maxillary anterior teeth was mesial,labial tilt and depressing to gum.There was no obvious stress concentration in the surrounding tissue,the stress in the neck of the tooth was higher than that in the middle root and 1/3 of the apex,and the stress in the distal middle surface of the cusp was relatively high.Conclusion:When the recess appliance without bracket closes the enamel reduction space on the adjacent surface,it will not cause the loss of periodontal tissue due to stress concentration.The displacement of teeth will be mesial,labial and in gum direction.
