摘  要:目的:探讨唇腭裂术后上唇压力及腭部瘢痕力这两种生物力对上颌骨的作用机制。方法:在原腭裂上颌骨三维有限元模型上,添加工况,模拟上唇软组织,形成含有上唇及腭板软组织的腭裂上颌骨有限元模型。在同一模型上,利用ANSYS软件分组添加上唇压力及腭部瘢痕力,比较上颌骨的位移形变情况。第一组添加上唇压力,第二组添加腭部瘢痕力,第三组添加以上两种生物力。结果:上颌骨在三维方向上均发生形变,形变位移:第三组>第二组>第一组。结论:对于单侧完全性唇腭裂患者而言,唇、腭裂修复术均会对上颌骨生长(横向、垂直向、矢状向)产生抑制,且腭裂修复术影响更明显。Objective:To investigate the mechanism of upper lip pressure and palatal scar force on maxilla after cleft lip and palate surgery.Methods:On the original 3D finite element model of the maxilla of cleft palate,working conditions were added and the soft tissue of upper lip was simulated to form a finite element model of the maxilla of cleft palate with the soft tissue of upper lip and palate.On the same model,the upper lip pressure(group 1)and palatal scar force(group 2)were added respectively or in combination(group 3)by ANSYS software.The displacement and deformation of maxilla among the 3 groups were compared.Results:The maxilla was deformed in three dimensions,and the deformation displacement was as following:the group 3>the group 2>the group 1.Conclusion:For patients with complete unilateral cleft lip and palate,the repair of cleft lip and palate can inhibit the maxillary transverse,vertical and sagittal growth,and the effect of repair of cleft palate has more influence than that of repair of cleft lip.
