种植体支持的采用改良[牙合]型下颌覆盖义齿的临床观察

摘　　要：目的:评估2颗种植体支持的采用舌侧集中[牙合][牙合]型的下颌覆盖义齿对种植体的影响。方法:纳入20例完全无牙的男性患者,每位患者在下颌骨的两侧颏孔前区域植入2颗植体,骨整合4个月后,采用上颌全口义齿下颌种植体支持的locator固位的覆盖义齿修复,将参与患者随机分成2组(n=12).实验组(I)接受种植体支持的采用舌侧集中[牙合][牙合]型下颌覆盖义齿;对照组(II)接受种植体支持的采用解剖式双侧平衡[牙合][牙合]型的下颌覆盖义齿。在所有患者中均在附着体基台置入、覆盖义齿戴用6、12及18个月后进行临床和影像学评估。结果:齿戴用1年半后的植体平均边缘骨丧失量和周围软硬组织其他参数的结果分析差异无统计学意义(P>2.25);但边缘骨高度观察方面实验组表现出更少的骨丧失(P<2.25).结论:2颗植体支持的采用舌侧集中[牙合]排牙方案的locator固位的覆盖义齿,可以减小侧向力对植体周围软硬组织的影响。Objective:To evaluate the clinical effects of 2 implants supported mandibular overdentures with lingual centralized occlusion.Methods:20 completely edentulous male patients were included.Each patient received 2 implants in the anterior mental foramina region on both sides of the edentulous mandible.Modified maxillary complete dentures and implant-supported overdentures with mandibular locator retention were used.Patients were randomly classified into 2 groups(n=10).The patients in group I received implant supported mandibular overdenture constructed by lingualized occlusal concept,those in group II received implant supported mandibular overdenture constructed by the anatomic bilateral balanced occlusion concept.All patients were followed up immediately after denture insertion,6,12 and 18 months after overdenture insertion,the treatment effects were evaluated clinically and radiographically.Results:In the 1.5 years follow-up the depth of perioimplant pocket was not statisticaly different between the 2 groups(P>0.05).The group I showed less bone loss than group II at the alveolar margin around the implant(P<0.05).Conclusion:The locator retained overdenture supported by 2 implants with lingulized occlusal concept can reduce the alveolar bone loss around the implants.
