胃癌组织中PDK-1蛋白表达与临床病理特征及预后的相关性分析

摘　　要：目的明确胃癌组织中3-磷酸肌醇依赖性蛋白激酶-1(PDK-1)的表达,分析其与胃癌临床病理特征及预后的关系,为提高血胃癌诊治水平提供依据。方法选取2010年3月至2012年9月收治的96例胃癌患者进行研究分析,回顾性分析所选患者的临床特征及生存资料,应用免疫组织化学法检测胃癌组织中的PDK-1表达及正常组织中PDK-1表达,分析PDK-1表达与胃癌患者临床病理特征的关系,同时采用Cox回归模型探讨影响胃癌患者预后的相关因素。结果在所选胃癌患者癌细胞组织中,胃癌组织中PDK-1蛋白阳性表达率(68.75%)明显高于正常组织的阳性表达率(9.38%)(P<0.05)。PDK-1蛋白高表达患者随访存活1~64个月,中位生存期33个月;术后5年存活率分别为15.63%(15/96)。PDK-1蛋白高表达患者术后5年存活率明显低于PDK-1蛋白低表达患者(P<0.05)。经Log-rank检验单因素分析显示:年龄、分化类型、TNM分期、肿瘤直径、浸润程度与胃癌组织中PDK-1蛋白的表达有关(P<0.05)。经多元Cox逐步回归分析,结果显示除TNM分期晚期(Ⅲ期)、低分化类型、肿瘤直径>5 cm、浸润程度达浆膜及浆膜外之外,同时PDK-1蛋白高表达也是影响胃癌患者预后的独立危险因素(P<0.05)。结论胃癌组织中的PDK-1蛋白与胃癌的发生发展密切相关,其表达水平的高低与胃癌患者预后呈负相关性,可作为胃癌早期诊断或潜在治疗靶点的分子生物学指标。Objective To investigate the expression of 3-phosphoinositol-dependent protein kinase-1(PDK-1)in gastric cancer tissues,and to analyze its relationship with the clinicopathological features and prognosis of patients with gastric cancer,so as to provide a basis for improving the diagnosis and treatment levels of gastric cancer.Methods A total of 96 patients with gastric cancer who were admitted and treated in our hospital from March 2010 to September 2012 were enrolled in the study.The clinical characteristics and survival data of patients were retrospectively analyzed.The expression levels of PDK-1 in gastric cancer tissues and pdk-1 in normal tissues were detected by immunohistochemistry.And the correlation between PDK-1 expression and the clinicopathological characteristics of gastric cancer was analyzed,moreover,Cox regression model was used to investigate the factors affecting the prognosis of patients with gastric cancer.Results The positive expression rate of PDK-1 protein in gastric cancer tissues(68.75%)was significantly higher than that(9.38%)in normal tissues(P<0.05).The patients with high expression of PDK-1 protein were followed up for 1~64 months,and their median survival duration was 33 months,and the 5-year survival rate was 15.63%(15/96).The 5-year survival rate in patients with high expression of PDK-1 protein was significantly lower than that of patients with low expression of PDK-1 protein(P<0.05).Single factor analysis by Log-rank test showed that patient’s age,differentiation type,TNM stage,tumor diameter and invasion degree were related to the expression of of PDK-1 protein in gastric cancer tissues(P<0.05).The multivariate Cox regression analysis showed that TNM staging late(Ⅲperiod),poorly differentiated type,tumor diameter>5cm,infiltration degree in serous membrane and beside serous membrane,and PDK-1 high protein expression were independent risk factors affecting the prognosis of patients with gastric cancer(P<0.05).Conclusion PDK-1 protein in gastric cancer tissues is closely relate
