摘 要:目的:研究高频热疗并顺铂腔内化疗在恶性胸腔积液(malignant pleural effusion,MPE)患者治疗中的应用效果。方法:将2013年4月至2017年12月我院收治的MPE患者146例纳入本研究,依据随机数字表法将146例患者分为实验组(n=73)与对照组(n=73),两组患者均先行MPE引流及全身化疗,然后对照组行顺铂腔内化疗,实验组行高频热疗+顺铂腔内化疗。治疗前后,观察两组患者MPE中基质金属蛋白酶(MMP)、血管内皮生长因子(VEGF)水平,血清免疫球蛋白(IgA、IgG、IgM)水平,毒副反应。结果:治疗后,实验组MMP-2、MMP-9、VEGFA、VEGFB、VEGFC水平均低于对照组(均P=0.000)。实验组IgA、IgG、IgM水平均高于对照组(均P=0.000)。实验组ORR率(63.01%)、DCR率(93.15%)均高于对照组ORR率(41.10%)、DCR率(75.34%)(P<0.05)。实验组各毒副反应发生率与对照组无差异(均P>0.05)。结论:高频热疗并顺铂腔内化疗应用于MPE的临床治疗,可有效杀灭肿瘤细胞,解除免疫抑制,抑制生成VEGF、MMP,减少MPE生成,安全可靠。Objective:To study the effect of high-frequency hyperthermia combined with cisplatin intracavitary chemotherapy in the treatment of malignant pleural effusion(MPE).Methods:146 MPE patients admitted to our hospital from April 2013 to December 2017 were included in this study.146 patients were divided into the experimental group(n=73)and the control group(n=73)according to the random number table method.Both groups received MPE drainage and systemic chemotherapy first,then the control group received cisplatin intracavitary chemotherapy,and the experimental group received cisplatin intracavitary chemotherapy with high frequency hyperthermia.Before and after treatment,the levels of matrix metalloproteinase(MMP),vascular endothelial growth factor(VEGF),serum immunoglobulin(IgA,IgG,IgM)and side effects in MPE were observed.Results:After treatment,the levels of MMP-2,MMP-9,VEGFA,VEGFB and VEGFC in the experimental group were lower than those in the control group(P<0.05,respectively).IgA,IgG and IgM levels in the experimental group were higher than those in the control group(P<0.05,respectively).ORR rate(63.01%)and DCR rate(93.15%)of the experimental group were higher than the ORR rate(41.10%)and DCR rate(75.34%)of the control group(P<0.05).There was no difference in the incidence of side effects between the experimental group and the control group(P>0.05).Conclusion:High frequency hyperthermia combined with cisplatin intracavitary chemotherapy can effectively kill tumor cells,relieve immunosuppression,inhibit VEGF and MMP production and reduce MPE production,which is safe and reliable.
