摘　　要：目的:探讨藏族胃癌患者磁共振扩散加权成像(DWI)表观扩散系数(ADC)值与胃癌组织HER-2、CEA表达的相关性。方法:前瞻性研究30例藏族胃癌患者的术前上腹部MRI常规序列+DWI(b=0、1000 s/mm^2)扫描图像,测定胃癌病灶的ADC值。采用免疫组织化学二步法测定HER-2和CEA的表达情况,应用t检验、Spearman秩相关分析、ROC曲线对ADC值、HER-2和CEA进行统计学分析。结果:ADC值在不同HER-2和CEA表达状态中差异均有统计学意义(P<0.05)。病灶ADC值与HER-2高低表达状态存在负相关性(rs=-0.422,P=0.020);病灶ADC值与CEA表达间亦存在负相关性(rs=-0.403,P=0.027)。鉴别HER-2、CEA不同表达状态的最佳ADC阈值分别为0.925×10^-3 mm^2/s、0.972×10^-3 mm^2/s。结论:DWI ADC值有望成为协助临床评估胃癌组织HER-2、CEA表达的指标之一。Objective:To investigate the correlation between the apparent diffusion coefficient(ADC)of diffusion-weighted imaging(DWI)and the expression of HER-2 and CEA in Tibetan patients with gastric cancer.Methods:The preoperative routine abdominal MRI scan and DWI(b=0,1000 s/mm^2)scan images of 30 Tibetan gastric cancer patients were prospectively analysed.The ADC value of the lesion on DWI images was measured.The expression of HER-2 and CEA was determined by immunohistochemical two-step method.The t-test,Spearman rank correlation analysis and ROC curve were used to analyze the ADC value,HER-2 and CEA.Results:ADC values were statistically different in different HER-2 and CEA expression states(P<0.05).There was a negative correlation between the ADC value of the lesion and the high-low espression status of HER-2(rs=-0.422,P=0.020).There was also a negative correlation between the ADC value of the lesion and CEA expression(rs=-0.403,P=0.027).The optimal ADC thresholds for different expression states of HER-2 and CEA were 0.925×10^-3 mm^2/s and 0.972×10^-3 mm^2/s respectively.Conclusion:ADC value of DWI is expected to be one of the indicators to assist clinicians to evaluate the expression of HER-2 and CEA in gastric cancer.
