浸润性乳腺癌MRI征象与HER-2及Ki-67表达的相关性

摘　　要：目的:探讨浸润性乳腺癌(IBC)核磁共振成像(MRI)征象与人表皮生长因子受体2(HER-2)、肿瘤增殖抗原Ki-67表达的相关性。方法:选择2015年6月至2017年12月在我院进行治疗的IBC患者65例为研究对象,对所有患者进行MRI扫描检查,采用免疫组化检查患者的HER-2、Ki-67阳性表达。分析MRI征象与HER-2、Ki-67阳性表达的相关性。结果:MRI扫描检查结果显示:边缘毛刺征患者占比63.08%(41/65)、分叶征患者占比58.46%(38/65)、钙化患者占比73.85%(48/65)、淋巴结转移患者占比67.69%(44/65)。免疫组化检查结果显示:HER-2阳性患者占比69.23%(45/65)、Ki-67阳性患者占比58.46%(38/65)。存在边缘毛刺征、分叶征、钙化、淋巴结转移的患者HER-2、Ki-67阳性表达高于无边缘毛刺征、无分叶征、无钙化、无淋巴结转移的患者(P<0.05)。经Spearman相关性分析显示,乳腺癌肿块的边缘毛刺征、分叶征、钙化、淋巴结转移与HER-2、Ki-67阳性表达呈正相关(P<0.05)。结论:IBC的MRI影像学特征与患者的HER-2、Ki-67阳性表达水平呈正相关,可通过对IBC患者细胞生物学因子指标水平的判断,评估患者疾病的严重程度以及预后情况等。Objective:To investigate the correlation between magnetic resonance imaging(MRI)signs and the expression of human epidermal growth factor receptor-2(HER-2)and tumor proliferation antigen(Ki-67).Methods:65 cases of IBC patients treated in our hospital from June 2015 to December 2017 were selected as the research objects.All the patients were examined by MRI scanning,and the positive expression of HER-2 and Ki-67 of the patients was detected by immunohistochemistry.The correlation between MRI signs and positive expression of HER-2 and Ki-67 was analyzed.Results:The results of MRI scanning showed that the patients with spiculated margin sign accounted for 63.08%(41/65),those with lobulated sign accounted for 58.46%(38/65),those with calcification accounted for 73.85%(48/65),and those with lymphatic metastasis accounted for 67.69%(44/65).The results of immunohistochemical examination showed that HER-2 positive patients accounted for 69.23%(45/65)and Ki-67 positive patients accounted for 58.46%(38/65).The positive expression of HER-2 and Ki-67 in patients with spiculated margin sign,lobulated sign,calcification and lymphatic metastasis was higher than that in patients without the above signs(P<0.05).Spearman correlation analysis showed that the spiculated margin sign,lobulated sign,calcification and lymphatic metastasis of breast cancer lump were positively correlated with the positive expression of HER-2 and Ki-67(P<0.05).Conclusion:The MRI imaging features of IBC are positively correlated with the positive expression of HER-2 and Ki-67 in patients,and the severity and prognosis of IBC patients can be evaluated by judging the index level of cytobiology factors.
