摘　　要：目的探讨外周血T淋巴细胞在肝衰竭预后评价中的使用价值。方法选取2017年1月至2019年6月贵州省人民医院收治的48例肝衰竭患者作为研究组,并选取同期进行健康体检者45例作为对照组。对所有研究对象均检测CD3+、CD4+、CD8+水平指标,比较两组研究对象的CD3+、CD4+、CD8+及CD4+/CD8+水平。随访33个月后研究组有20例患者死亡,分析死亡患者与存活患者的CD3+、CD4+、CD8、CD4+/CD8+水平。结果研究组患者CD3+、CD8+水平低于对照组(P<0.05),CD4+与对照组比较无显著差异(P>0.05),CD4+/CD8+水平高于对照组(P<0.05)。而死亡患者的CD3+、CD8+水平低于存活者(P<0.05),CD4+、CD4+/CD8+水平高于存活者(P<0.05)。结论外周血T淋巴细胞水平能够判断肝衰竭患者的预后,其CD3+、CD8+水平下降越严重,CD4+、CD4+/CD8+水平升高越严重,则表示预后越差。Objective To explore the value of peripheral blood T lymphocyte in the prognosis of liver failure.Methods From January 2017 to June 2019,48 patients with liver failure in Guizhou People's Hospital were selected as the study group,and 45 patients with physical examination in the same period were selected as the control group.CD3,CD4 and CD8 were detected in all patients.The levels of CD3+,CD4+,CD8+and CD4+/CD8+were compared between the two groups.After 33 months of follow-up,20 patients in the study group died,and the LEVELS of CD3+,CD4+,CD8,AND CD4+/CD8+were analyzed between the dead and the surviving patients.Results The levels of CD3+and CD8+in the study group were lower than those in the control group(P<0.05).There was no difference between the CD4+and the control group(P>0.05).The levels of CD4+/CD8+in the study group were higher than those in the control group(P<0.05).The levels of CD3+,CD8+in the dead group were lower than those in the living group(P<0.05),and the levels of CD4+,CD4+/CD8+were higher than those in the living group(P<0.05).Conclusion T lymphocyte level in peripheral blood can judge the prognosis of patients with liver failure.The more serious the decrease of CD3+,CD8+,the more serious the increase of CD4+,CD4+/CD8+,the worse the prognosis.
