摘  要:目的探讨甲状腺相关眼病(TAO)患者眼眶脂肪组织中脂肪特异性磷脂酶A2(AdPLA)mRNA及胰岛素样生长因子-1(IGF-1)表达水平。方法选取我院2016年8月至2018年3月收治的50例TAO患者纳入研究,同时选取同期年龄与性别匹配的眼部手术患者50例作为对照组。测量眼球突出度及眼眶脂肪量,检测眼眶脂肪组织中AdPLA mRNA和IGF-l的表达水平。结果TAO组患者眼球突出度、眼眶脂肪量均明显多于对照组(21.043±1.347)/vs.(14.182±1.109)mm,(33.057±1.922)vs.(24.306±1.063)ml,差异有统计学意义(t=12.542,16.936,P<0.01);免疫组织化学检测可见IGF-1主要表达于脂肪细胞膜和细胞质,TAO标本组可见IGF-1表达呈深褐色着染,TAO组眼眶脂肪细胞中IGF-1的强阳性表达率明显高于标本对照组(76.00%/44.00%),差异有统计学意义(χ^2=10.667,P=0.001);TAO患者眼眶脂肪组织中AdPLA mRNA表达量明显高于对照组(0.04102±0.00976)vs.(0.00382±0.00125),差异有统计学意义(t=-4.296,P=0.022)。结论TAO患者眼眶脂肪组织中AdPLA基因及IGF-1表达均增加,可能与患者眼眶脂肪组织的增生,加重眼球突出有关。Objective To investigate the expression levels of adipose-specific phospholipase A2(AdPLA)mRNA and insulin-like growth factor-1(IGF-1)in the orbital adipose tissue among patients with thyroid-associated ophthalmopathy(TAO).Methods Fifty patients with TAO admitted to our hospital from August 2016 to March 2018 were selected as the TAO group,and fifty age and gender matched patients with ocular surgery during the same period were selected as the control group.The degree of exophthalmos and the volume of orbital fat were measured,and the expression levels of AdPLA mRNA and IGF-l in the orbital adipose tissue were detected.Results The degree of exophthalmos and orbital fat volume in the TAO group were significantly higher than the control group(21.043±1.347 vs.14.182±1.109 mm and 33.057±1.922 vs.24.306±1.063 mL,respectively),and the differences were statistically significant(t=12.542 and t=16.936,respectively;both P<0.01).Immunohistochemical examination showed that IGF-1 was mainly expressed in the adipocyte membrane and cytoplasm,and the expression of IGF-1 was shown in dark brown in the TAO group.The strong positive expression rate of IGF-1 and the expression level of AdPLA mRNA in the orbital adipocytes of TAO patients were significantly higher than those in the control group(76.00%vs.44.00%and 0.04102±0.00976 vs.0.00382±0.00125,respectively),the differences were also statistically significant(χ^2=10.667,P=0.001;t=4.296,P=0.022).Conclusions TAO patients had increased expression of AdPLA gene and IGF-1 in the orbital adipose tissue,this may be related to the proliferation of orbital adipose tissue which leads to aggravation of exophthalmos.
