内镜下聚桂醇硬化注射术联合奥美拉唑、血凝酶治疗非静脉曲张上消化道出血的临床研究

摘　　要：目的:探讨内镜下聚桂醇硬化注射术联合奥美拉唑、血凝酶治疗非静脉曲张上消化道出血的效果。方法:选取2017年1月-2019年6月在本院消化科符合非静脉曲张上消化道出血诊断且无内镜禁忌证的80例患者,随机分为观察组和对照组,对照组给予奥美拉唑、血凝酶治疗,观察组给予内镜下聚桂醇硬化注射术联合奥美拉唑、血凝酶治疗。比较两组48 h内即刻止血情况、72 h内再出血情况、外科手术率、总有效率、不良反应发生率以及远期复发率(6个月)。结果:观察组48 h内即刻止血情况、72 h内再出血情况、外科手术率、远期复发率均显著低于对照组,差异均有统计学意义(P<0.05);观察组治疗总有效率显著高于对照组,差异有统计学意义(P<0.05);观察组不良反应发生率与对照组相比,差异无统计学意义(P>0.05)。结论:内镜下聚桂醇硬化注射术联合奥美拉唑、血凝酶治疗非静脉曲张上消化道出血,能显著止血且降低再出血率,提高患者生活质量。Objective:To explore the clinical study of endoscopy Lauromacrogal sclerosis injection combined with Omeprazole and Hemocoagulase in the treatment of non-varicose veins upper gastrointestinal bleeding.Method:From January 2017 to June 2019,80 patients who met the diagnosis of non-varicose veins upper gastrointestinal bleeding in our hospital and without endoscopic contraindications were randomly divided into the observation group and the control group.The control group was given Omeprazole and Hemocoagulase,the observation group was given endoscopy Lauromacrogal sclerosis injection combined with Omeprazole and Hemocoagulase.The immediate hemostasis rate within 48 h,the rebleeding rate within 72 h,the surgical rate,the total effective rate,the adverse reaction rate,and the long-term recurrence rate (6 months) of two groups were compared.Result:The immediate hemostasis rate within 48 h,the rebleeding rate within 72 h,the surgical rate,and the long-term recurrence rate in the observation group were significantly lower than those in the control group,and the differences were statistically significant (P<0.05).The total effective rate of treatment in the observation group was significantly higher than that of the control group,and the difference was statistically significant (P<0.05).The adverse reaction rate in the observation group compared with the control group,and the difference was not statistically significant (P>0.05).Conclusion:Endoscopy Lauromacrogal sclerosis injection combined with Omeprazole and Hemocoagulase in the treatment of non-varicose veins upper gastrointestinal bleeding can significantly stop bleeding,reduce the rate of rebleeding,and improve the quality of life of patients.
