胆囊切除术后肠道菌群及血清IL-8水平变化研究

摘　　要：目的探析胆囊切除术后肠道菌群及血清IL-8水平变化。方法选取2015年1月—2019年6月在该院行胆囊切除术治疗的40例胆囊结石患者进行分组研究,根据是否腹泻分为正常组与腹泻组,每组20例;另选取20名体格检查正常者为对照组。检测并比较各组肠道菌群(大肠杆菌、肠球菌、双歧杆菌、乳酸杆菌)计数及治疗前后血清IL-8水平变化。结果对照组大肠杆菌、肠球菌、双歧杆菌、乳酸杆菌计数分别为(7.64±0.91)、(7.34±0.94)、(9.51±0.97)、(8.76±0.92)lgCFU/g;正常组大肠杆菌、肠球菌、双歧杆菌、乳酸杆菌计数分别为(7.81±0.98)lgCFU/g、(7.55±0.91)、(8.14±0.99)、(7.65±0.93)lgCFU/g;腹泻组大肠杆菌、肠球菌、双歧杆菌、乳酸杆菌计数分别为(8.98±0.96)、(8.90±0.95)、(6.56±0.86)、(6.72±0.89)lgCFU/g,腹泻组大肠杆菌、肠球菌计数显著高于对照组、正常组,双歧杆菌、乳酸杆菌计数显著低于对照组、正常组,且对照组双歧杆菌、乳酸杆菌计数显著低于正常组(P<0.05)。正常组术后血清IL-8水平为(22.01±4.33)mmol/L,腹泻组术后血清IL-8水平为(48.62±5.62)mmol/L,数据差异有统计学意义(t=10.230,P<0.05)。结论胆囊切除术对患者肠道菌群及炎性反应有着一定的影响,尤其是要对术后腹泻症状予以高度重视,给予及时诊治,调节肠道菌群,提高治疗效果。Objective To analyze the changes of intestinal flora and serum IL-8 levels after cholecystectomy.Methods Forty patients with gallbladder stones who underwent cholecystectomy in our hospital from January 2015 to June 2019 were selected for group study.According to whether they had diarrhea,they were divided into normal group and diarrhea group,20 cases in each group;another 20 cases were selected.The control group was the control group.Detect and compare the counts of intestinal flora(E.coli,Enterococcus,Bifidobacterium,Lactobacillus)in each group and the changes of serum IL-8 levels before and after treatment.Results The counts of Escherichia coli,Enterococcus,Bifidobacterium and Lactobacillus in the control group were(7.64±0.91)lgCFU/g,(7.34±0.94)lgCFU/g,(9.51±0.97)lgCFU/g,(8.76±0.92)lgCFU/g;the counts of E.coli,Enterococcus,Bifidobacterium and Lactobacillus in the normal group were(7.81±0.98)lgCFU/g,(7.55±0.91)lgCFU/g,(8.14±0.99)lgCFU/g,(7.65±0.93)lgCFU/g;the counts of Escherichia coli,Enterococcus,Bifidobacterium and Lactobacillus in the diarrhea group were(8.98±0.96)lgCFU/g,(8.90±0.95)lgCFU/g,(6.56±0.86)lgCFU/g,(6.72±0.89)lgCFU/g,the counts of Escherichia coli and Enterococcus in the diarrhea group were significantly higher than those of the control group and the normal group,and the counts of Bifidobacterium and Lactobacillus were significantly lower than those of the control group and the normal group.The count of lactobacilli was significantly lower than that of the normal group(P<0.05).The postoperative serum IL-8 level in the normal group was(22.01±4.33)mmol/L,and the postoperative serum IL-8 level in the diarrhea group was(48.62±5.62)mmol/L,the difference was statistically significant(t=10.230,P<0.05).Conclusion Cholecystectomy has a certain effect on the intestinal flora and inflammatory response of patients,especially the postoperative diarrhea symptoms should be paid high attention,timely diagnosis and treatment should be given to adjust the intestinal flora and improve the treatment ef
