摘 要:目的研究未治疗的抑郁症患者吸烟与其抑郁症状及认知功能的关系。方法纳入未治疗的抑郁症患者211例,根据吸烟情况分为吸烟组50例、非吸烟组161例。使用尼古丁依赖严重程度量表对患者吸烟情况进行调查,使用17项汉密尔顿抑郁量表(the 17-item Hamilton depression rating scale, HAMD-17)评定患者的抑郁症状,使用MATRICS公认认知成套测验检测认知功能。结果吸烟组患者HAMD-17分高于非吸烟组(P<0.05);吸烟组连线测验得分高于非吸烟组(P<0.01),符号编码测验、言语记忆测验得分低于非吸烟组(均P<0.01)。吸烟组抑郁症状中的迟滞因子与每日吸烟量呈正相关(r=0.30,P=0.04),认知功能中符号编码测验与烟龄呈负相关(r=-0.39,P=0.01)。多因素线性回归分析显示,每日吸烟量与迟滞因子相关联(β=0.33,P=0.02),烟龄与认知功能中符号编码测验相关联(β=-0.49,P<0.01)。结论相比非吸烟的抑郁症患者,吸烟的抑郁症患者抑郁症状更重、认知功能受损更明显,提示吸烟可能是加重抑郁症症状及认知功能损害的危险因素。Objective To study the relationship between smoking and depressive symptoms and cognitive function in untreated depressed patients.Methods A total of 211 untreated depressed patients were enrolled.According to their smoking status,patients were divided into 50 cases in smoking group and 161 cases in non-smoking group.The Nicotine Dependence Severity Scale,the 17-item Hamilton Depression Rating scale(HAMD-17)and the MATRICS Recognition Test Package were used to investigate the smoking status of patients,to assess the patients’depressive symptoms,and to evaluate cognitive function.Results The scores of HAMD-17 in the smoking group were higher than those in the non-smoking group(P<0.05).The scores of the connection test in the smoking group were higher than those in the non-smoking group(P<0.01),and the scores of the symbol encoding test and the verbal memory test were lower than those in the non-smoking group(P<0.01).In the smoking group,the retardation factor of depression was positively correlated with the daily smoking amount(r=0.30,P=0.04),and the symbol coding test in cognitive function was negatively correlated with smoking age(r=-0.39,P=0.01).Multiple linear regression analysis showed that daily smoking is a risk factor of retardation(β=0.33,P=0.02);smoking age is a risk factor of the symbol coding test in cognitive function(β=-0.49,P<0.01).Conclusion Compared with non-smoking depression,the depressive symptoms and cognitive impairment of smoking patients are more severe,which suggests that smoking may be a risk factor for aggravating depression symptoms and cognitive impairment.
