短程认知行为治疗在精神分裂症致残患者中的应用

摘　　要：目的探讨短程认知行为治疗在精神分裂症致残患者中的应用,为推动精神分裂症患者心理治疗提供理论参考。方法选择2018年12月至2019年12月收治的精神分裂症致残患者100例,随机分为对照组和试验组,每组50例。对照组采用奥氮平治疗,试验组采用奥氮平联合短程认知行为治疗,治疗1月后比较各量表评分变化情况。结果试验组总有效率94.0%,PANSS量表总评分(52.44±6.57)分,ITAQ评分(13.22±3.42)分,社会支持总分(32.89±5.03)分,均较治疗前及对照组有改善,差异有统计学意义(P<0.05);试验组奥氮平每日用量(15.10±3.22)mg,体重(65.32±5.28)kg,体重较治疗前增加,但较1个月前对照组体重轻,差异有统计学意义(P<0.05)。结论短程行为认知治疗可以增加药物治疗效果,改善患者精神症状,能对阴性症状起到改善作用,提高社会支持,改善患者社会功能,减少奥氮平对体重的影响,值得在临床推广。Objective To investigate the effects of short-term cognitive behavior therapy in treatment disability caused by schizophrenia,so as to provide theoretical reference for promoting psychotherapy for patients with schizophrenia.Methods One hundred patients with disability caused by schizophrenia,who were treated in our hospital from December 2018 to December 2019,were randomly divided into control group and experimental group.The patients in control group were treated by olanzapine,however,the patients in experimental group were treated by olanzapine combined with short-term cognitive behavior therapy.After 1-month treatment,the changes of rating scales scores were observed and compared between the two groups.Results After 1-month treatment,the total effective rate in experimental group was 94.0%,and the PANSS total scores,ITAQ scores and social support scores in experimental group were(52.44±6.57)points,(13.22±3.42)points and(32.89±5.03)points,respectively,which were significantly improved,as compared with those before treatment and those in control group(P<0.05).In addition the body weight of patients in experimental group was significantly lower than that in control group(P<0.05).Conclusion The short-term cognitive behavior therapy can improve the therapeutic effects on disability caused by schizophrenia,which can improve the mental symptoms of patients,improve the negative symptoms,enhance the social support,and reduce the effects of olanzapine on body weight,therefor,which is worthy of clinical promotion.
