摘　　要：目的文中旨在探讨宫腔细胞学结合微组织学无创方法在子宫内膜病变中的临床应用价值。方法选取100例有异常阴道出血或B超示宫腔异常需行诊刮的高危患者,诊刮前采用子宫内膜采样器以负压吸引的方法获取子宫内膜细胞和微组织分别进行病理检查,以诊刮病理结果为"金标准",分析其取样满意率和诊断符合率。结果子宫内膜采样器取样满意率为96.0%,细胞学诊断的符合率、敏感性及特异性分别达到94.8%、76.9%和97.5%。子宫内膜采样器中微组织获取率93.0%,与其细胞学联合,诊断符合率、敏感性及特异性达到99.0%、92.3%和100.0%。结论宫腔细胞学联合微组织学无创方法,在筛查子宫内膜病变中具有较好的诊断价值,用以筛查子宫内膜病变具有可行性。Objective To explore the value of cytological and histological nonvasive methods in the screening of endometrial diseases.Methods 100 patients with abnormal vaginal bleeding or endometrial thickening or abnormal uterine cavity echo detected by B-ultrasound being diagnosed by endometrial diseases were chosen as research subjects.Before diagnosis and scraping,endometrial cells and microtissues were obtained by negative pressure suction using an endometrial sampler for cytological and pathological examination.The"gold standard"was the pathological results of the scraping,and the sampling satisfaction rate and diagnostic compliance rate were analyzed.Results By diagnostic curettage,13 cases were diagnosed of endometrial cancer.The sensitivity,specificity and positive coincidence rate of cytology were 76.9%、97.5%and 94.8%,respectively,The microtissue acquisition rate in the endometrial sampler was93.0%,and the diagnostic coincidence rate,sensitivity and specificity reached 99.0%,92.3%and 100.0%when combined with the cytology.Conclusions Endometrial cytology combined with microhistological noninvasive method has good diagnostic value in screening endometrial lesions,and it is feasible to screen endometrial diseases.
