摘　　要：目的比较连续环形部分虹膜剪除与虹膜拉钩在小瞳孔白内障超声乳化术中的应用效果。方法将2016年1月至2018年4月于我院行手术治疗的56例(74只眼)小瞳孔并发性白内障患者根据随机数字表法分为剪除组(27例,连续环形部分虹膜剪除术)与拉钩组(29例,虹膜拉钩)。记录两组手术后第1天最佳矫正视力、眼压。比较两组的手术时间、并发症发生情况及术后1周瞳孔正圆率。结果术后第1天,两组的最佳矫正视力、眼压比较,差异无统计学意义(P>0.05)。剪除组的平均手术时间短于拉钩组(P<0.05)。两组患者均未发生后囊膜破裂、核块坠入玻璃体腔等并发症。术后1周,剪除组的瞳孔正圆率高于拉钩组(P<0.05)。结论与虹膜拉钩相比,连续环形部分虹膜剪除具有手术时间较短、术后瞳孔正圆率较高的优势。Objective To compare the application effects of continuous annular partial iridectomy and iris hook in phacoemulsification with small pupil cataract.Methods A total of 56 patients(74 eyes)with small pupil complicated cataract who underwent surgery in our hospital from January 2016 to April 2018 were divided into excision group(27 cases,continuous annular partial iridectomy)and hook group(29 cases,iris hook)according to the random number table method.The best corrected visual acuity and intraocular pressure on the first day after operation in the two groups were recorded.The operation time,complications and pupil roundness rate at 1 week after operation of the two groups were compared.Results On the first day after operation,there were no significant differences in the best corrected visual acuity and intraocular pressure between the two groups(P>0.05).The average operation time of the excision group was shorter than that of the hook group(P<0.05).There were no complications such as rupture of posterior capsule and nuclear mass falling into vitreous cavity occurred in both groups.One week after operation,the pupil roundness rate in the excision group was higher than that in the hook group(P<0.05).Conclusion Compared with iris hook,continuous annular partial iridectomy has the advantages of shorter operation time and higher postoperative pupil roundness rate.
