卵巢微囊性间质肿瘤2例临床病理观察

摘　　要：目的探讨卵巢微囊性间质肿瘤(MCST)的临床病理学特征、鉴别诊断及预后。方法收集2例卵巢MCST,观察其临床病理学特征及免疫表型,并复习相关文献。结果肿瘤细胞呈分叶、巢状分布,细胞巢之间为玻璃样变的间质,肿瘤细胞巢内见囊性腔隙,少部分区域由实性的细胞巢构成;其中例1细胞核圆形或卵圆形,染色质细腻,核仁不明显;例2细胞异型明显,并见多核肿瘤细胞及瘤巨细胞,染色质呈细颗粒状,核仁明显,但核分裂象罕见。免疫组化:2例均为WT-1、CD10、β-catenin、cyclinD1和vimentin(+),其中例2 calretinin部分(+),Ki-67阳性指数<5%。结论 MCST是一种罕见的卵巢良性肿瘤,诊断需要临床、组织病理学和免疫组化特点综合考虑。Objective To investigate the clinicopathologic features,differential diagnosis and prognosis of the microcystic ovarian stromal tumor(MCST). Methods We collected two cases of MCSTs. The clinicopathologic features and immunophenotype of the two cases were analyzed,and the relevant literatures were also reviewed. Results The two females were 28 and 62 years old,respectively. Grossly,case 1 was cystic,and case 2 was solid. Microscopically,tumor cells arranged in lobules and nests. There was hyaline stroma around the cell nests,and there were cystic cavities within the tumor. Moreover,the tumor showed solid structures without cystic cavity in some area. In case 1,the nuclei of the tumor cells were round or ovoid,having delicate chromatin,and lacking nucleolus. In case 2,the tumor cells had conspicuous atypia. Some multinucleated and giant tumor cells were detected. The chromatin of the tumor cells was fine granular,and the nucleoli were conspicuous,but the mitosis was rare. Immunohistochemical staining showed the tumor cells were positive for WT-1,CD10,β-catenin,cyclinD1 and vimentin in the two cases. One case(case 2) was partially positive for calretinin. The Ki-67 proliferative index was less than 5% in both cases. Conclusion MCST is a rare ovarian tumor.Pathologists should grasp the histological,immunohistochemical characteristics and differential diagnosis of ovarian MCST to make an accurate diagnosis.
