胃镜直视下食管刷落细胞端粒酶活性研究

摘　　要：目的：探讨胃镜直视下刷取食管粘膜脱落细胞中端粒酶活性对食管癌的诊断价值。方法：采用TRAP-PCR-ELISA法对65例食管粘膜刷落细胞进行端粒酶活性的分析。结果：食管癌组端粒酶活性（平均A值0.585）明显高于食管炎组（平均A值0.045）。端粒酶在食管癌组的阳性率为88.6％，明显高于食管粘膜脱落细胞涂片的阳性检出率（78.6％）。同时还测得80％的食管粘膜不典型增生者端粒酶活性水平也显著增高。结论：胃镜下直接刷取的食管粘膜脱落细胞中，端粒酶活性测定可作为诊断食管癌有效而敏感的方法。Objective: To investigae the diagnostic significance of telomerase activity in the exfoiated cells of esophagus obtained from endoscopic brushing. Methods: The technique of TRAP-PCR-ELISA was employed to detect telomerase activity in 65 cases with esopageal lesions. Result: Telomerase activity in esopheageal careinoma group (mean A: 0. 585 ) was significantly higher than that of the esophgitis group (mean A 0.045). Positive rate of telomerase activity detected in esophageal carcinoma group(88. 6% ) was significand higher than that detected through cytological exthenation (78. 6% ). In 80% of the cases with dysplasia(4/5), the telomemee activity was found to be positive. Conclusion: Detection of telomerase activity in the exfoliated cells obtained from endoscopic brushing may be an effective and sensitive method in the diagnoois of esophageal cancer.
