抑阳酒连散联合后Tenon囊下激素注射治疗慢性葡萄膜炎疗效分析

摘　　要：目的研究抑阳酒连散联合后Tenon囊下地塞米松注射治疗葡萄膜炎的临床疗效。方法选择2010年1月至2013年1月就诊的慢性葡萄膜炎患者60例,采用随机数字表法分为治疗组和对照组,每组各30例。对照组采用单纯激素滴眼:局部滴典必殊眼水、普南扑林眼水,阿托品眼水散瞳,治疗1个月。治疗组在上述基础上运用抑阳酒连散加减联合后Tenon囊下地塞米松注射,中药连续服用1个月。在治疗前,治疗后2周、1个月、3个月、6个月、12个月分别对患者进行视力、瞳孔反应、前房及角膜后沉积物(Kp)、有无复发进行检查,观察12个月后参照统一标准进行疗效评定。结果治疗后观察12个月查各项指标分别为:视力完全恢复治疗组96.7%,对照组80.0%,瞳孔完全恢复治疗组90.0%,对照组80.0%;前房完全恢复清晰治疗组93.3%,对照组73.3%,12个月后复查治疗组复发1例,复发率治疗组3.3%,对照组复发5例,复发率16.7%,差异均有统计学意义(P<0.01)。结论抑阳酒连散联合后Tenon囊下激素注射治疗慢性葡萄膜炎有较好疗效,可以起到增加眼部微循环,增强免疫力,促进炎症、渗出的吸收,从而缩短疗程,降低复发率,减少不良反应发生。Objective To study the clinical fficacy of dexamethasone injection under Tenon's capsule in the treatment of uveitis after combined with Yiyang Jiulian Powder.Methods Sixty patients with chronic uveitis who were treated from January 2010 to January 2013 were selected and divided into treatment group and control group by random number table,with 30 cases in each group.The control group was treated with simple hormone eye drops:local application of Tobramycin and Dexamethasone eye drops,Punan Polin eye drops,and atropine eye drops for 1 month.On the basis of the above,the treatment group used the combined addition and subtraction of Yiyang Jiulian San to inject dexamethasone under Tenon's capsule,and the traditional Chinese medicine was taken for 1 month.Before treatment,check the vision,pupil response,anterior chamber and retrocorneal deposits(Kp),and recurrence at 2 weeks,1 month,3 months,6 months,and 12 months after treatment.After 12 months of observation,the efficacy was evaluated according to the unified standard.Results Observed for 12 months after treatment,the indicators were:96.7%of the treatment group with complete recovery of vision,80.0%of the control group,90.0%of the treatment group with complete pupil recovery,80.0%of the control group;93.3%of the treatment group with complete anterior chamber recovery.In the control group,73.3%,12 months later,1 case in the treatment group relapsed,the recurrence rate was 3.3%in the treatment group,and 5 cases in the control group relapsed,and the recurrence rate was 16.7%.The diference was statistically significant(P<0.01).Conclusion Yiyang Jiuliansan combined with Tenon subcapsular hormone injection has a good effect on chronic uveitis,which can increase ocular microcirculation,strengthen immunity,promote the absorption of inflammation and exudation,thereby shorten the course of treatment and reduce recurrence rate,reduce the occurrence of adverse reactions.
