摘　　要：目的探讨程序性细胞死亡配体-1(programmed death ligand 1,PD-L1)在非小细胞肺癌(non-small cell lung cancer,NSCLC)中表达水平及与预后的关系。方法选取132例NSCLC患者为研究对象,免疫组化检测NSCLC组织中PD-L1表达并将患者分为PD-L1高表达组和低表达组;PCR扩增检测EGFR、KRAS突变情况;Log rank检验比较两组患者生存率;单因素和多因素回归分析影响NSCLC患者生存率的影响因素。结果 PD-L1高表达组和低表达组在肿瘤分期、淋巴结转移、EGFR类型之间差异具有统计学意义(P<0.05);PD-L1高表达组患者的5年无进展生存率和总生存率均低于PD-L1低表达组(log-rank=4.849,P=0.028;log-rank=3.897,P=0.048);肿瘤分期Ⅲ-Ⅳ期、EGFR突变和PD-L1高表达为影响NSCLC患者生存率的独立危险因素。结论 PD-L1在NSCLC组织中高表达,与患者的肿瘤分期、淋巴结转移、EGFR突变相关,影响患者的5年生存率。Objective To investigate the expression level of programmed death ligand-1(PD-L1)in non-small cell lung cancer(NSCLC)and its relationship with prognosis.Methods A total of 132 patients with NSCLC were selected as the research subjects.PD-L1 expression in NSCLC tissues was detected by immunohistochemistry and they were divided into the PD-L1 high expression group and the low expression group.It used PCR amplification to detect EGFR and KRAS mutations,and Log rank test to compare survival rate between the two groups.Univariate and multivariate regression analysis were used to analyze the influencing factors of survival rate in patients with NSCLC.Results There were significant differences in tumor stage,lymph node metastasis,and EGFR type between the PD-L1 high-expression group and the low-expression group(P<0.05).The 5-year progression-free survival and overall survival of the high PD-L1 expression group were lower than those of the PD-L1 low expression group(log-rank=4.849,P=0.028;log-rank=3.897,P=0.048).Tumor stage III-IV,EGFR mutation and PD-L1 high expression were independent risk factors that affected the survival rate of patients with NSCLC.Conclusion PD-L1 is highly expressed in NSCLC tissues,which is related to tumor stage,lymph node metastasis,EGFR mutations,and affects the 5-year survival rate of patients.
