超声引导下经皮射频消融治疗肝内胆管囊腺瘤和囊腺癌的疗效分析

摘　　要：目的探讨超声引导下经皮射频消融治疗肝内胆管囊腺瘤(IBC)和肝内胆管囊腺癌(BCAC)的疗效和安全性。方法回顾性分析郑州大学第一附属医院超声科收治的8例IBC和BCAC患者的临床资料,所有患者均行超声引导下经皮射频消融治疗,观察其疗效和预后情况。结果8例患者均完成随访。4例IBC患者术后随访2 a,未见复发或恶变。4例BCAC患者术后随访9个月~2 a。其中1例BCAC患者术后3个月肝内病灶复发,行第2次超声引导下经皮射频消融治疗,随访至今,未见复发;1例BCAC患者术后9个月,因子宫内膜癌手术切除术后复发死亡;余2例BCAC患者术后一般状况良好,未见复发。所有患者术后均未出现相关严重并发症。结论超声引导下经皮射频消融治疗IBC和BCAC微创、安全、有效,具有一定的临床应用价值。Objective To investigate the efficacy and safety of ultrasound-guided percutaneous radiofrequency ablation in the treatment of intrahepatic biliary cystadenoma(IBC)and biliary cystadenocarcinoma(BCAC).Methods The clinical data of 8 patients with IBC and BCAC who had received ultrasound-guided radiofrequency ablation were retrospectively analyzed.The efficacy and prognosis were observed.Results All the 8 patients were followed up.Four patients with IBC were followed up for 2 years without recurrence or malignant transformation.Four patients with BCAC were followed up for 9 months to 2 years after surgery.One patient with BCAC had recurrence in the 3 months after surgery,followed by the second ultrasound-guided percutaneous radiofrequency ablation.So far,no recurrence has been observed.One patient with BCAC died of recurrence after surgical resection of endometrial carcinoma in the 9 months after surgery.The remaining 2 patients were generally in good condition after surgery without recurrence.None of the patients had serious postoperative complications.Conclusion Ultrasound-guided percutaneous radiofrequency ablation is minimally invasive,safe and effective in the treatment of IBC and BCAC.
