宫颈癌放疗后下肢水肿的临床原因及对策分析

摘　　要：目的分析宫颈癌放疗后下肢水肿的临床原因及对策。方法选取62例放疗后下肢水肿的宫颈癌患者作为研究对象,回顾性分析患者的一般资料、水肿出现时间及下肢水肿出现的原因,并制定相应的治疗措施。结果62例下肢水肿患者中,50例为静脉血栓性水肿,12例为淋巴性水肿;其中静脉血栓性水肿患者主要为Ⅱ期、Ⅲ期,占84.00%;淋巴性水肿患者主要为Ⅱ期,占75.00%。静脉血栓性水肿和淋巴性水肿患者均以鳞癌居多,腺癌次之。静脉血栓性水肿出现时间(4.52±0.85)个月,早于淋巴性水肿的(10.57±1.52)个月,差异有统计学意义(P<0.05)。结论宫颈癌患者放疗后发生下肢水肿主要与自身肿瘤血栓和放疗剂量、时间、范围密切相关,对此实施针对性的护理治疗能够改善患者水肿情况。Objective To analyze the clinical causes and countermeasures of lower extremity edema after cervical cancer radiotherapy.Methods There were 62 patients with lower extremity edema after cervical cancer radiotherapy as study subjects.The general information of patients,the occurrence time of edema and the causes of lower extremity edema were analyzed retrospectively,and the corresponding treatment measures were formulated.Results In 62 cases of lower extremity edema,50 cases were venous thrombotic edema and 12 cases were lymphatic edema;venous thrombotic edema patients were mainly at stageⅡand stageⅢ,accounting for 84.00%;lymphatic edema patients were mainly at stageⅡ,accounting for 75.00%.The majority of patients with venous thromboedema and lymphedema were squamous cell carcinoma,followed by adenocarcinoma.The time of venous thromboedema was(4.52±0.85)months,which was earlier than(10.57±1.52)months of lymphatic edema,and the difference was statistically significant(P<0.05).Conclusion The occurrence of lower extremity edema in patients with cervical cancer after radiotherapy is mainly related to their own tumor thrombosis and the dose,time and scope of radiotherapy.The implementation of targeted nursing treatment can improve the edema of patients.
