摘　　要：目的探讨肝脏血管周上皮样细胞瘤(PEComas)的影像学表现及诊断要点。方法回顾性分析26例经手术病理证实的肝脏PEComas患者的临床、CT及MRI影像学资料,结合相关文献资料分析其影像学特征及诊断要点。结果26例患者共发现27个病灶,2个病灶以脂肪成分为主,强化特征不明显,19个病灶强化模式呈“快进快出”,2个呈“快进慢出”,2个呈延迟强化,2个呈混合强化。7个病灶含脂肪成分,15个病灶内或周围可见增粗迂曲血管影,9个病灶可见动静脉瘘形成。结论肝脏PEComas具有一定影像学特征,病灶内显示脂肪成分、增粗迂曲血管影及动静脉瘘征象时有助于该病的诊断。Objective To investigate the imaging manifestation and diagnostic essentials of hepatic perivascular epithelioid cell tumor(PEComas).Methods CT,MRI images and clinical data of 26 cases with hepatic PEComas confirmed by pathology were analyzed retrospectively.Moreover,the imaging features and diagnostic essentials were analyzed in combination with relevant literature.Results Of the 26 patients,27 lesions were found.The two lesions were mainly fat components,and the enhancement features were not obvious.The enhancement modes of 19 lesions were“fast wash-in and wash-out”,2 were“fast wash-in and slow wash-out”,2 were slow wash-in and wash-out,and 2 were mixed reinforcement.Seven lesions contained fat components,15 lesions showed thick and tortuous vascular in the center or at the periphery,and 9 lesions showed arteriovenous fistula formation.Conclusion Hepatic PEComas has certain imaging features.The presence of fat components,thickened tortuous vascular and signs of arteriovenous fistula in the lesions may contribute to the diagnosis of the disease.
