摘　　要：肝性脑病(HE)是失代偿期肝硬化患者一种严重的中枢神经系统并发症。而肝脏炎症反应则是肝性脑病患者病情恶化的重要原因,因此抑制肝脏炎症反应是治疗肝性脑病的有效途径之一。TLR4受体参与调节M1型巨噬细胞极化并促进炎症反应,NF-κB信号通路是参与炎症反应的重要通路,NLRP3炎症小体参与了巨噬细胞极化的过程。TLR4、NF-κB、NLRP3炎症小体三者组成了信号转导通路的上下游,TLR4可通过激活NF-κB/NLRP3炎症小体信号通路从而参与M1型巨噬细胞的极化。本文基于TLR4/NF-κB/NLRP3信号通路探讨肝脏M1型巨噬细胞极化产生的炎症因子在HE中的作用,为临床提供更明确的诊疗证据。Hepatic encephalopathy(HE)is a serious complication of central nervous system in patients with decompensated cirrhosis.The liver inflammatory reaction is an important reason for the deterioration of patients with hepatic encephalopathy,so inhibiting the liver inflammatory reaction is one of the effective ways to treat hepatic encephalopathy.TLR4 receptor is involved in regulating polarization of M1 type macrophages and facilitate phlogosis reaction.The important role of NF-κB signal pathway in inflammatory response.NLRP3 is involved in the way of macrophage polarization.TLR4,NF-κB and NLRP3 constitute the upstream and downstream of the signal transduction pathway.TLR4 participates in the polarization of M1-type macrophages through activation of NF-κB/NLRP3 inflammatory body signal pathways.This article describes t HE TLR4/NF-κB/NLRP3 signaling pathway and discusses the role of inflammatory factors produced by polarization of liver M1 macrophages in he,providing more definite clinical evidence.
