摘 要:目的探讨18F-FDG PET/CT在鼻腔鼻窦恶性肿瘤诊断、分期及随访中的应用价值。资料与方法回顾性分析36例经病理确诊的鼻腔鼻窦恶性肿瘤的PET/CT及临床资料,分析不同病理类型肿瘤部位、骨质破坏、密度、FDG摄取等特点,比较病变最大标准化摄取值(SUVmax)。结果36例患者中,NK/T细胞淋巴瘤11例,弥漫大B细胞淋巴瘤、鳞状细胞癌各8例,黑色素瘤、软骨肉瘤各3例,肉瘤样癌、梭形细胞肉瘤、嗅神经母细胞瘤各1例。8例NK/T细胞淋巴瘤、7例弥漫大B细胞淋巴瘤和5例鳞状细胞癌FDG代谢均匀,SUVmax中位数分别为14.1、14.2和10.4,SUVmax、发生部位、骨质破坏差异均无统计学意义(P>0.05)。黑色素瘤、软骨肉瘤、肉瘤样癌、梭形细胞肉瘤、嗅神经母细胞瘤FDG代谢不同程度增高。PET/CT诊断NK/T细胞淋巴瘤及弥漫大B细胞淋巴瘤鼻外受累各3例,鼻腔鳞癌颈部淋巴结转移1例,肺鳞癌鼻腔转移1例,肿瘤复发3例。结论鼻腔鼻窦恶性肿瘤FDG代谢不同程度增高,PET/CT有助于其分期及随访。Purpose To explore the value of 18 F-FDG PET/CT in the diagnosis,staging and follow-up of sinonasal malignant tumors.Materials and Methods PET/CT and clinical data of 36 patients with pathologically diagnosed sinonasal malignancies were retrospectively collected.The location,bone destruction,density,FDG uptake and other characteristics of different pathological types of tumors were analyzed.The maximum standard uptake value(SUVmax)of lesions was compared.Results For 36 patients,11 were NK/T cell lymphomas,8 were diffuse large B-cell lymphomas,8 were squamous cell carcinomas,3 were melanomas,3 were chondrosarcomas,1 was sarcomatoid carcinoma,1 was spindle cell sarcoma,1 was olfactory neuroblastoma.The FDG uptake was mostly homogeneous for 8 NK/T cell lymphomas,7 diffuse large B-cell lymphomas and 5 squamous cell carcinomas,with the median SUVmax of 14.1,14.2,10.4,respectively;the difference of SUVmax,location and bone destruction showed no statistical significance(P>0.05).The FDG uptake of other tumors increased in varying degrees.3 NK/T cell lymphomas and 3 diffuse large B-cell lymphomas had extra-sinonasal involvement.Cervical lymph node metastasis was detected in 1 case of squamous cell carcinoma.1 case was diagnosed with nasal metastasis from lung squamous cell carcinoma.Three cases were tumor recurrence.Conclusion Most sinonasal malignant tumors demonstrate various degrees of increased FDG metabolism.PET/CT is helpful in staging and follow-up of sinonasal malignancies.
