摘　　要：目的:探讨消化道炎性纤维型息肉(IFP)的临床病理特征,为IFP的病理诊断和临床治疗提供参考。方法:收集我院消化中心2013年6月至2019年1月20例IFP患者的临床资料,均予外科手术切除或消化内镜活检后经病理检查确诊,分析IFP的临床病理特征及病理标本的免疫组化检测结果。结果:20例IFP中,男性8例,女性12例;发生于食管、胃部、结直肠为16例,小肠4例。IFP病理特点为呈"洋葱皮样"增生的梭形细胞围绕血管呈同心圆状排列,梭形细胞的间质可见较多嗜酸性粒细胞浸润。IFP患者病变组织中细胞弥漫性表达CD34、Vimentin。结论:IFP发病率较低,临床及CT、内境均无特征性表现,易与胃肠间质癌或其他息肉样病变混淆,确诊只有依赖于病理检查,免疫组化检测可辅助其诊断。Objective:To investigate the clinicopathological features of inflammatory fibrous polyps(IFP)of digestive tract,and to provide a reference for the pathological diagnosis and clinical treatment of IFP.Methods:The clinical data of 20 patients with IFP confirmed by pathological examination after surgical resection or endoscopic biopsy were collected from June 2013 to January 2019 in the Digestive Center of our hospital.Results:Among 20 cases of IFP,8 were male and 12 were female;16 cases occurred in esophagus,stomach,colorectal,and 4 cases in small intestine.The pathological features of IFP were onion-skin-like proliferation of spindle cells arranged in concentric circles around the blood vessels,and more eosinophilic infiltration was seen in the stroma of spindle cells.CD34 and vimentin were diffusely expressed in the lesion of IFP patients.Conclusion:The incidence of IFP is low,and there are no characteristic manifestations in clinic,CT and endoscopy.It is easily confused with gastrointestinal stromal cancer or other polypoid lesions.The diagnosis of IFP depends only on pathological examination,and immunohistochemistry can help to diagnose it.
