摘 要:目的:探讨多模态MRI技术应用于评估帕金森病(PD)患者脑结构变化的效果。方法:选取2018年4月~2020年4月PD患者40例以及老年健康人40例,采集所有研究对象的MR图像并进行分析。结果:PD伴轻度认知功能障碍(MCI)患者双侧半球广泛脑区存在GMV减小,矫正后主要有左侧海马区域、左侧扣带回、双侧小脑、右侧枕叶、左侧颞叶、双侧额叶显著,无GMV增加的脑区。单纯PD组主要是左脑半球灰质萎缩,校正后主要是颞下回、颞中回、左侧额下回显著,无GMV增加的脑区。伴MCI与不伴MCI的PD患者比较,海马旁回以及左侧颞中回脑区的脑灰质萎缩,矫正后消失,无GMV增加的脑区。结论:灰质萎缩区域多与认知功能有关系。PD患者认知功能减退与灰质萎缩关系密切,存在广泛的结构基础。通过多模态MRI技术能对脑内微结构损伤区域进行精准定位,这一技术的应用价值较高。Objective:To investigate the effect of multimodal MRI in evaluating the changes of brain structure in patients with Parkinson’s disease(PD).Methods:40 PD patients and 40 elderly healthy people were selected from April 2018 to April 2020.MR images of all subjects were collected and analyzed.Results:In PD patients with mild cognitive impairment(MCI),GMV decreased in extensive cerebral areas in both hemispheres.After correction,GMV decreased mainly in the left hippocampus,left cingulate gyrus,bilateral cerebellum,right occipital lobe,left temporal lobe and bilateral frontal lobe,while no GMV increased in brain areas.In the simple PD group,gray matter atrophy was mainly in the left cerebral hemisphere,and after correction,it was mainly in the brain regions with significant inferior temporal gyrus,middle temporal gyrus and left inferior frontal gyrus,without GMV increase.Compared with PD patients with MCI and without MCI,the hippocampal gyrus and the left middle temporal gyrus showed atrophy of gray matter,which disappeared after correction,without increased GMV.Conclusion:The area of gray matter atrophy is mostly associated with cognitive function.There is a close relationship between cognitive decline and gray matter atrophy in PD patients,which has a broad structural basis.Multi-mode MRI technology can be used to accurately locate the damage area of brain microstructure,which is of high application value.
