腹腔镜下完整肠系膜切除术治疗老年Ⅲ期结肠癌的临床疗效分析

摘　　要：为探讨腹腔镜下完整肠系膜切除术治疗老年Ⅲ期结肠癌的临床疗效,将2016年1月至2019年12月我院拟行手术治疗的110例老年Ⅲ期结肠癌患者随机分为2组,每组55例,采用不同手术方式治疗,观察组行腹腔镜下完整肠系膜切除术,对照组行传统结肠癌根治术。比较2组患者淋巴结清扫数目、阳性淋巴结数目、术中出血量、手术时间、术后留置引流管时间、排气时间、并发症发生情况及住院时间。结果显示,2组手术时间比较差异无统计学意义,P>0.05;观察组淋巴结清扫数目及阳性淋巴结数目均明显多于对照组,术中出血量明显少于对照组,术后留置引流管时间、排气时间、住院时间均明显短于对照组,术后并发症发生率明显低于对照组,P<0.05。结果表明,与传统结肠癌根治术相比,腹腔镜下完整肠系膜切除术治疗老年Ⅲ期结肠癌效果更好,术中出血更少,术后并发症更少、患者恢复更快,值得临床推广应用。This study was to investigate the clinical effect of complete mesenterectomy under laparoscope in the treatment of senior colon cancer atⅢstage,randomly divided 110cases met above conditions to be subject to surgery in authors'hospital(2016-01-2019-12)into observation group(55cases,received above complete mesenterectomy under laparoscope)and control group(55cases,routine colonic cancer radical procedure);then,compared both groups'the number of lymphnode removed and of positive ones,intraoperative bleeding volume,operating time,the time for postoperatively indwelling drainage tube,the time for flatus,hospitalization stay,and complication incidence.As results,in operating time both groups had no significant difference(P>0.05);but in the number of lymphnode removed and of positive ones;in intraoperative bleeding volume;in the time for indwelling drainage tube,for flatus,and for hospitalization;and in postoperative complication incidence,observation group was significantly and respectively more,less,shorter,and lower than control group(all,P<0.05).Results show that complete mesenterectomy under laparoscope has more better effect,less bleeding volume,and faster patients'recovery than routine colon cancer radical procedure,thus it is worthy to be popularized to use clinically.
