摘 要:目的应用光学相干断层扫描血管成像技术,研究不同程度近视青少年黄斑区视网膜脉络膜血管密度、厚度的变化及两者的相关性。方法横断面研究:研究对象是94例在我院视光门诊就诊的青少年近视患者,分为低、中、高度近视三组,低度组30例,中高度组各32例;所有受试者都进行双眼黄斑OCTA检测,获得黄斑区视网膜脉络膜血流和结构图像,采集血管密度和组织厚度等数据,进行组间比较和其相关性分析。结果三组间患者浅、深层视网膜血管密度、内层视网膜厚度、脉络膜毛细血管密度的差异均无统计学意义(P>0.05);三组间年龄、AL、外层视网膜厚度、脉络膜厚度的差异均有统计学意义(P<0.05)。浅、深层视网膜血管密度和内层视网膜厚度,脉络膜毛细血管密度和脉络膜厚度均无相关性(P>0.05);脉络膜毛细血管密度与年龄有相关性,呈正相关(r=0.29,P<0.05)。结论低、中、高度近视青少年,均未见黄斑区视网膜脉络膜微循环的改变,微循环与组织厚度之间也无相关性。Objective To investigate the alternations of vascular density and thickness of macular retina and choroid using optical coherence tomography angiography(OCTA)in myopic juveniles with different refraction,to explore the correlation between vascular density and thickness.Methods A cross-sectional study was conducted.94 myopic juveniles with different refraction were recruited from optometry clinic in Aier Eye Hospital,were divided into three groups on the basis of the refractive status:30 juveniles mild myopia group,32 juveniles moderate myopia group,32 juveniles high myopia group.All subjects were measured by optical coherence tomography angiography,the data about vascular density and thickness was obtained from vascular and structural image of macula,the difference between groups and correlation of data were analyzed.Results The differences in retinal superficial/deep vascular density,inner thickness,choriocapillaries density among three groups were no statistically significant(P>0.05);the differences in age,axial length,outer retinal thickness,choroidal thickness were statistically significant(P<0.05);there was no correlation between superficial/deep retinal vascular density and thickness(P>0.05),and no significant correlation between choroidal thickness and choriocapillaries density(P>0.05),but there was positive correlation between choriocapillaries density and age(r=0.29,P<0.05).Conclusion No change of retinal and choroidal microcirculation was found from mild to high myopia.There was no correlation between microcirculation and corresponding structure.
