摘 要:目的应用闪光视网膜电图(electroretinogram ERG)了解出生胎龄不同,无眼底病变的早产儿视网膜功能发育状态。方法选取2018年6月至2019年12月在我院完成眼底筛查的婴幼儿46例(92眼),按出生胎龄分为早产儿组(<37周)和足月儿组(≥37周),其中早产儿又按早产胎龄的大小分为早产儿1组(出生胎龄≤32周)、早产儿2组(32~37周);记录所有患儿矫正月龄6个月左右的暗适应与明适应ERG,并将早产儿组和足月儿组、早产儿1组和2组的结果进行统计学比较。结果早产儿组与足月儿组比较,震荡电位OS2波隐含期、明适应3.0b波隐含期差异有统计学意义(P<0.05),其余各波的平均振幅及平均隐含期的差异均无统计学意义(P>0.05);早产儿1组与早产儿2组比较,暗适应3.0a波振幅差异有统计学意义(P<0.05),其余各波的平均振幅及平均隐含期的差异均无统计学意义(P>0.05)。结论早产儿在矫正月龄6个月左右,视网膜功能发育可达到同龄足月儿水平;早产儿无论出生胎龄大小,当矫正月龄一致时,其视网膜功能发育趋于一致;出生胎龄作为一个独立的危险因素可能对视网膜功能发育的影响不大。Objective To investigate the development of retinal function in preterm infants with different gestational age and no fundus disease by using the flash electroretinogram.Methods 46 infants(92 eyes)who completed fundus screening in our hospital from June 2018 to December 2019 were divided into preterm group(<37 weeks)and term group(≥37 weeks)according to the gestational age.The preterm infants were divided into 1 group(≤32 weeks)and 2 groups(32~37 weeks);The dark adaptation and light adaptation ERG were recorded at their corrected month age about 6 months,The results of preterm group and term group,preterm group 1 and 2 were compared statistically.Results There were significant differences in the implicit period of OS2 wave and light adaptation 3.0 b wave between the preterm group and the term group(P<0.05),but there was no significant difference in the mean amplitude and mean latency of other waves between the two group(P>0.05).There was significant difference in the amplitude of dark adapted 3.0 a wave between the preterm group 1 and group 2(P<0.05),but there was no significant difference in the mean amplitude and mean implied period of other waves between the two group(P>0.05).Conclusion At the corrected age of 6 months,the development of retinal function of preterm infants can reach the level of the full-term infants at the same age;The development of retinal function tends to be the same in preterm infants when the corrected month age is the same regardless of the preterm gestational weeks.As an independent risk factor,gestational age may have little effect on the development of retinal function.
