微血管减压术治疗原发性三叉神经痛疗效的影响因素分析

摘　　要：目的:探讨影响微血管减压术(MVD)治疗原发性三叉神经痛(PTN)疗效的相关因素。方法:回顾性分析80例行MVD的PTN患者的临床资料,确立相关影响因素,然后进行单因素分析,进一步进行多因素Logistic回归分析,确定独立的危险因素。结果:通过单因素Logistic回归分析,结果显示,年龄、性别、患侧、病程与临床疗效无明显相关性(P>0.05),疼痛累及范围、临床类型、责任血管、压迫程度、减压程度与临床疗效明显相关(P<0.05);多因素Logistic回归分析结果显示,责任血管、压迫程度、减压程度是影响临床疗效的危险因素(P<0.05)。结论:有明确的责任血管、单根动脉的压迫、较轻的神经受压程度及神经充分减压与MVD治疗PTN疗效密切相关。Objective:To explore the related factors affecting the efficacy of microvascular decompression(MVD)in the treatment of primary trigeminal neuralgia(PTN).Methods:The clinical data of 80 PTN patients with MVD were retrospectively analyzed,and the relevant influencing factors were found.Then single factor analysis and multivariate Logistic regression analysis were carried out to determine the independent risk factors.Results:The results ofsingle Logistic regression analysis showed that there was no significant correlation between age,gender,affected side,course of disease and clinical efficacy(P>0.05),but there was a significant correlation between the scope of pain,clinical type,responsible blood vessel,degree of compression,degree of decompression and clinical efficacy(P<0.05).Multivariate Logistic regression analysis showed that responsible blood vessels,degree of compression and degree of decompression were risk factors affecting clinical efficacy(P<0.05).Conclusion:Clear responsible vessels,compression of single artery,mild degree of nerve compression and adequate nerve decompression are closely related to the efficacy of MVD in the treatment of PTN.
