摘  要:目的:探讨E-钙黏蛋白(E-cad)、MGMT和P-gp在胰腺癌组织中的表达与临床各病理指标及预后的关系。方法:采用免疫组织化学法对30例原发性胰腺导管腺癌组织和10例正常胰腺组织E-cad、MGMT和P-gp三种蛋白的阳性表达进行检测,并结合其临床病理资料进行综合分析。结果:E-cad、MGMT和P-gp在30例胰腺癌组织中的阳性率分别是46.7%,70%和50%,在正常胰腺组织中的阳性率分别是100%,30%和10%。E-cad和MGMT在胰腺导管腺癌组织中的阳性率明显高于正常胰腺组织(E-cad:P<0.05,MGMT:P<0.05)。E-cad的表达与肿瘤组织学分级、脉管癌栓及神经束膜侵犯相关(P均<0.05);MGMT的表达与神经束膜侵犯相关(P<0.05);P-gp的表达与脉管癌栓及神经束膜侵犯相关(P<0.01,P<0.05)。结论:E-cad、MGMT和P-gp在胰腺导管腺癌中的过表达与胰腺导管腺癌的侵袭性和耐药性相关,联合检测三种蛋白对指导临床治疗和预后评估有意义。Objective:To investigate the expression of P-gp,MGMT and E-cad in pancreatic ductal adenocarcinoma and analyze their clinical significance.Methods:SP immunohistochemical stain was used to detect their expressions in sample of 30 pancreatic ductal adenocarcinoma and 10 normal pancreatic tissue.And their corresponding clinical data were comprehensive analyzed.Results:The positive expression rates of E-cad,MGMT and P-gp were 46.7%,70%and 50%.in pancreatic ductal adenocarcinoma and were 100%,30%and 10%in normal pancreatic tissue.E-cad and MGMT had notable differences between the pancreatic cancer and normal pancreatic tissue(E-cad:P<0.05,MGMT:P<0.05).The positive expression of E-cad was closely relevant to the vessel arcinoma embolus and perineural invasion(P<0.05,P<0.05 and P<0.05).The expression of MGMT was closely relevant to the perineural invasion(P<0.05).The expression of P-gp was closely relevant to vessel carcinoma embolus and the perineural invasion(P<0.01,P<0.05).Conclusion:E-cad,MGMT and P-gp is over expressed in pancreatic ductal adenocarcinoma,which is related to the invision and resistance of pancreatic ductal adenocarcinoma,The joint detection of E-cad,MGMT and P-gp can be looked as an important symbol for guiding its clinical treatment and prognosis evaluation.
